Sixth grade students require a physical examination and seventh grade students require a dental examination. If your child has had these examinations, please be sure to send a copy to the Nurse's office. Forms are available on the PV website. Your cooperation with this is most appreciated!
Ill students
Any student who is feeling ill during the school day must visit the health suite. They should not contact a parent on their own without seeing the nurse.

If your child requires medication during the school day, please be sure to bring the medication to school with the appropriate forms completed by the physician. Forms can be found on the PV website. No medication should be in the student’s possession (pockets, lunch, purse/handbag, or backpack). All medications are to be delivered directly to the nurse’s office and kept in the health room unless otherwise designated by the school nurse.
