**IMPORTANT – PLEASE READ ALL INSTRUCTIONS
Registering for high school sports involves completing the online athletic registration at Form RELeaf as well has turning in a sports physical.
Form RELeaf is required for each sports season. Only one Sports Physical is required each school year.
HCPS Medical Eligibility Preparticipation Physical Evaluation Form is REQUIRED – Sports Physical forms packet is 4 PAGES – Please note - Only the HCPS Medical Eligibility Preparticipation Physical Evaluation Form comes to School Nurse.
Date of Exam MUST BE AFTER JUNE 7, 2023 – no exceptions.

- Pages 1 and 2 - Preparticipation Physical Evaluation – History Form (2 pages) – Done by parent/guardian and athlete. This form is kept on file with the provider or parent can keep.
- Page 3 - Preparticipation Physical Evaluation - Physical Examination Form (1 page) Done by the Healthcare Provider AND kept on file with the provider.
Page 4 - HCPS MEDICAL ELIGIBILITY Preparticipation Physical Evaluation (1 page) – Done by Healthcare Provider AND Parent/Guardian. Provider must complete top part and sign and parent must complete bottom part and sign. This form is the only form that is turned in to the School Nurse. (1 page) All other forms stay w/provider or parent.
HCPS MEDICAL ELIGIBILITY PREPARTICIPATION PHYSICAL EVALUATION FORM IS DUE BEFORE The Beginning Of The 1st Sports Season The Athlete Wants To Join (One Per School Year) - Form must be turned in at the school BEFORE start date of the sport. If form is turned at the first tryout, there could be a delay in your student starting Fall Sports due to forms processing.
WHERE TO TURN FORM IN - There will be a desk with a YELLOW Box on it outside of Health Suite 1 (Main Hall)as each sports season approaches. Please put ONLY the HCPS Medical Eligibility Preparticipation Physical Evaluation Form in the box. School Nurse will be processing the forms as they come in. If the box is not in hallway, please give your physical directly to the School Nurse.
Email of form is not preferred. If you email, we still need original for clearance.
MEDICATIONS – If your student requires the use of an emergency medication such as an inhaler for asthma or epinephrine injection (like EpiPen or Auvi-Q) for severe allergy, please make sure to have the Self Carry Medication Form - 2 PAGES** completed by your Healthcare Provider and turned in with HCPS Medical Eligibility Form. Students must be authorized by School Nurse to carry any medication. **These forms must be turned in prior to student participation.

Where Can Students Get A Sports Physical?
Sports Physicals are different than Annual Medical Physicals done at the Pediatrician’s Office. A sports physical focuses on reviewing your child's current health status and medical history to ensure that your child is healthy enough to play his or her sport. Sports Physicals are usually not covered by insurance. Note: All exams must be dated after June 7, 2023. There are several options to where a Sports Physical can be done:
NOTE: TAKE HCPS Sports Physical Packet WITH YOU TO PROVIDER DOING THE SPORTS PHYSICAL!
Urgent Care Centers – Sports Physicals are most often done at Urgent Care Centers. The exam and paperwork is done at same time. Cost is $15 and up (check with the center your are going to when you schedule or walk in). Take the Sports Physical Forms Packet with you!
CVS/Target Minute Clinic – Sports Physicals can be done at CVS/Target Minute Clinic. The exam and paperwork is done at same time. Cost is $15 and up (check with the location you are going to when you schedule or walk in). Take the Sports Physical Packet with you!
Pediatrician/Primary Care Provider – Sports Physicals can be done at your student’s primary care provider/pediatrician’s office. If your provider is filling out the forms (provide them with the Sports Physical Packet) it is VERY IMPORTANT that that exam date and date on form is AFTER JUNE 7, 2023. (Many providers will fill out forms, but the form is based from an exam* prior to filling out the forms. *Exam date MUST BE after June 7, 2023. Costs for Sports Physicals by pediatrician/primary care provider varies by provider.
