The school district requires four (4) pieces of information for your child to have medication administered at school.
Physician medication order (attached)
Allergy and/or asthma action plan filled out by your child’s doctor
Signed parent consent (attached)
Your child’s medication brought to the school labeled with your child’s name, in the original container

- New Day, New Location for 2021 - Sunday, Capping Downtown Block Party Weekend
- Sunday, August 29 | 11 AM - 4 PM
- Centennial Green at Village Hall
- Downtown Mount Prospect, Intersection of Emerson Street and Busse Avenue
The festival will have a cultural taste of Mount Prospect, Cultural Booths, Info Booths, Kids Crafts, and of course Cultural Entertainment. This is a partnership between the Village, the Mt. Prospect Park District, and the Mount Prospect Chamber of Commerce.
