November 8th: Scoliosis screening for 4K-2nd grade
January 18th: Vision and Hearing Screening 4K
April 9th: Tdap vaccines for 6th graders

Quick Facts About Epilepsy
1 in 10 people will have a seizure and 1 in 26 will develop epilepsy during their lifetime.
There are 3.4 million people living with epilepsy in the United States; 470,000 of them are children.
Epilepsy is a neurological disorder that can affect any one of us regardless of race, age, or gender.
Seizures are a symptom of epilepsy. Not all seizures are the same. Many people with epilepsy have more than one type of seizure.
HELP NEEDED
Medication at School
- If your child/children will need to take prescription medication during the school day a completed medication authorization form MUST BE signed by their provider, prior to any medication be given.
- If your child/children will need to take over the counter medication while at school, please fill out the medication authorization form and turn it into the school nurse.
- All medication must be brought to school by a parent or in a lock box/bag and given to the school nurse.
- If you are in need of a lock box/bag for transporting medication to school, please contact Nurse Mandy.
Please note a new form is needed for all medications each school year.
Health Plans
Health Plans are required to be updated yearly. If your child/children have had a health plan in the past an updated plan will be needed for the 2023-2024 school year.
If your child/children has a chronic health condition they may require an individualized health plan, please contact the school nurse for more information.
Health plans directly written by the doctor's office are accepted.

The program is open to all students in the district 4K through 12th grade.
