5Essentials Survey
Share your thoughts to help improve our schools!
D140 students, teachers, and parents will be given the opportunity to have a voice in improving their schools by taking the annual statewide Illinois 5Essentials Survey from February 3 through April 2, 2021.

These surveys will serve as an opportunity for schools to reflect on progress since the last statewide implementation. School and District leaders use their 5Essentials data to better inform their school improvement plans and help identify key strengths and areas for growth. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.

5Essentials Student Survey
Students will take the 5Essentials Survey during their asynchronous time starting the week of February 22nd. The link for the survey will be posted on their teacher's Google Classroom pages. Students will need their state ID number and full birthdate to take the survey. Hybrid students will be given the slip with their state id number while at school. Remote students will receive an email with instructions on how to take the survey. Exact instructions on how to access the survey will be sent to students next week.
