A Message from the School Clinic
ATTENTION: PARENTS OF STUDENTS WHO HAVE MEDICATION AT SCHOOL
If your student has medications in the school Clinic, please stop in to pick them up on or before May 29th, 2019. We are unable to store medications over the summer and medications remaining in the Clinic will be discarded.
If your student will require medication during the school day or has a health condition that will require special care during the upcoming school year:
Visit the Mayfield Schools website at www.mayfieldschools.org , Click on Families and then scroll down to and click on Student Health Services. Along the right side will be forms to download and print for your student’s specific needs.
After these forms have been completed by you and your healthcare provider, they can be returned to the school via fax at 440-995-7405 or you can email them to email@example.com
Please have all forms and medications at the school by 8/15/19 so that your child is all
set for the first day of school.
If you have questions, please contact the Clinic at your child’s school until 5/29/19.

2019-2020 Supply Kits
CSA will be offering to the families of Center School the convenience of ordering School Supply Kits for the 2019-20 School Year. We have collaborated with 1st Day School Supplies to help parents and teachers get the exact supplies needed for next school year.
The kits have been specifically tailored for the families of Center School directly from the supply lists distributed by the school. See below for more details on how to place an order, due dates and teacher requests. Supply lists are available in the Friday Folder section of the Center School Website for those of you who prefer to buy your own supplies.
Backpacks, lunchboxes and water bottles can also be ordered from 1st Day School Supplies.
Sale Dates: May 1st- 31st
1. Orders can be made online until Friday, May 31st
The website for ordering will be shutdown at Midnight and no additional orders will be accepted.
2. Follow these steps to place your order:
Step 1: Enter our School Zip Code in the Start Here box. (School Zip Code is 44143)
Step 2: Select Supply kits
Step 3: Select the grade you need to order for and proceed as needed
PICK-UP – August 6th: An email will be sent the week prior to confirm the date and a pick-up time will be provided.
IMPORTANT: Please refer to the specific school supply list for the grade your child is entering on the Center School website for the following:
1. Labeling of student materials. Some teachers may request you to NOT label all or some of the items.
2. Specialized items such as paint shirts, tote bags, picture/pattern folders, etc. may not be available through 1st Day School Supplies

YEAR END EVENTS ARE RIGHT AROUND THE CORNER
FIELD DAY - FRIDAY, MAY 24th
Field day will be run similar to the Center Sprint. Volunteers are needed and greatly appreciated for this event! The rain date is May 28th. Click here to sign up to volunteer for field day!
5TH GRADE ACTIVITIES - WEDNESDAY, May 29th
At 1:00 we will have an assembly in the gym with 5th grade students and their parents. A video of photos will be shown, and students will receive their "diploma."
At 2:15 pm, the 5th grade students will parade by each classroom. The clap out will finish with the 5th graders exiting and returning to their classrooms for parent photos.
SCHOOL PICNIC - Wednesday, MAY 29th
Our school picnic for grades K-5 will be held on Wednesday, May 29th. Parents are invited to join us for a school picnic that will be held in the back of the school building on the playground.
