Prosper ISD will be closed June 24th-July 9th. There will be no access to PHS during these weeks.

Per UIL Rules, all students (woodwind, brass, percussion and color guard members) that are participating in the Prosper Mighty Eagle Band must have a sports physical completed and submitted by August 1st in order to participate in the Marching Season. A physical is only required every other year of participation.

Please upload BOTH PAGES of the completed physical form. The page labeled "PREPARTICIPATION PHYSICAL EVALUATION -- MEDICAL HISTORY" is to be filled out and signed by a parent or legal guardian and the second page entitled "PREPARTICIPATION PHYSICAL EVALUATION -- PHYSICAL EXAMINATION" is to be filled out by an authorized medical professional.

Physicals can be scheduled through your family physician or also at any CVS or Walgreens with a clinic.
