Please Complete Forms Below
Below this section, you will find some forms for you to complete and information to help your student and your family to navigate the year. The emergency form is essential for your student. It allows us to distribute OTC medication and reach other contacts in case of an emergency. In the past, we would have parents complete this on paper. This year we have an easier electronic form. Below the emergency form, you will find links to other forms and documents. For ones that you need to submit, please either print or make a copy, complete the form, sign it, and send it in with your child. If you have any questions, please let us know at 508-432-4500 ext 2000
For the 2021-22 school year, we are offering an extra layer of protection to prevent the spread of COVID-19, our COVID-19 testing program at school. This program has two components: symptomatic testing for students who show symptoms of COVID-19 at school and test and stay for students who may have been exposed while in school but are not showing symptoms. You will find information below in multiple languages to help explain the program.
Yesterday Massachusetts Education Commissioner Jeffrey C. Riley received the authority to mandate masks for all public K-12 students, educators, and staff through October 1 to ensure schools fully reopen safely and to provide time for more students and educators to get vaccinated. After October 1, the commissioner’s policy would allow middle and high schools to lift the mask mandate for vaccinated students and staff only if the school meets a certain vaccination rate – at least 80 percent of students and staff in a school building are vaccinated. Unvaccinated students and staff would still be required to wear masks. DESE also updated a Mask Mandate for the first few weeks. Cape Cod Tech is close but has not hit the 80% threshold. If a few more students get the vaccine, then we may meet that 80% threshold by October 1st.
