We Need your Input! Yearly 5 Essentials Survey for Parents/Students and Teachers.
Dear Parents/Guardians,
Beginning November 15th, students and teachers across Illinois will have an opportunity to participate in the seventh annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of your child’s school. As a parent, the survey window is now open and this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school. On behalf of the Illinois State Board of Education, the Illinois 5Essentials Survey will be administered online by UChicago Impact at the University of Chicago. The survey gathers data related to five indicators that can predict important student outcomes, including improved attendance and larger test score gains. These five indicators that affect and predict school success are: Effective Leaders, Collaborative Teachers, Involved Families, Supportive Environments, and Ambitious Instruction
Prior research in more than 400 schools has shown that schools that were strong on at least three of these “5Essential” indicators were 10 times more likely to improve student learning gains in math and reading than those that were weak on three or more Essentials. All teachers and our 4th through 8th-grade students will be responding to this survey. 5Essentials Reports will be generated for schools if their teachers and/or students meet the response rate threshold of 50 percent. These reports will be sent to schools and districts by May 2019 and will also be included in the State School Report Card. If you do not want your child to take the survey, please contact your child’s school.
Your participation in the parent portion of the survey will help us understand the conditions at your child’s school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child. The Illinois 5Essentials Survey for students and teachers will be conducted November 15, 2018, through January 18, 2019. The Parent Survey will be conducted through January 18, 2019. To take the survey please visit http://survey.5-essentials.org/illinois/survey/parent/.
For more information about the Illinois 5Essentials Survey, or to view previous years’ reports, please visit https://www.5-essentials.org/illinois. If you have any questions or concerns, please contact 5Essentials Client Services at 1-866-440-1874 or 5essentials@uchicago.edu.
Thank you for your support.

Art Show-LAST CALL FOR ART SHOW ENTRIES
Does your child love to draw, paint, and create beautiful things in general? Please consider having them submit their work they have done outside of school to the OQMS student art show and fundraiser. It will be held on Dec. 4th and will be at the Lemont High School this year. We are excited that the high school invited our students to show their work at their gallery.
All OQMS students are welcome to submit their work provided it is ORIGINAL and in good condition. Miss Dorrance is willing to help students mount, trim or finish their work to have it ready to hang in the show during advisory.
All work must be submitted with the permission slip and entry fee to Miss Dorrance by November 14. This will allow time to make labels and get work to the high school to hang.
Please contact Miss Dorrance with any questions at jdorrance@sd113a.org or 630-257-2286 x4259. If for any reason you need an extra day to send in your child's work, please contact MIss Dorrance.
