Immunization Clinic for JUNIORS 2-1-18
The clinic will take place in the Special Events Center at 8:30am. Please direct any questions to our nurse, Jill Scott, at jill.scott@nkcschools.org, 816-321-6955. All forms are attached below the letter as well as on the school website.
Dear Parent/Guardian,
Keeping your student immunized is important to your child’s health and each year the Clay County Public Health Center provides thousands of immunizations to students from all over Clay County.
This year, the Clay County Public Health Center will be providing required and recommended school immunizations for students attending Staley High School on February 1, 2018 beginning at 8:30am. The vaccines being offered are: Menactra, Men B, HPV, and FLU. Vaccine information sheets on all vaccines available are provided for your review.
If you would like your student to receive any of these vaccines, please complete the front and back of the form and send a copy of your insurance card (front and back) with the completed consent. All students can receive this service regardless of insurance coverage. The consent must be completed and returned to the school nurse by Thursday, January 25. Please encourage your student to have breakfast the day of the clinic.
Thank you for allowing the Clay County Public Health Center to be a partner in your child’s health. If you have any questions, please feel free to contact your school nurse.
