Welcome to the 2023-24 School Year!
Hello Jane Addams Families,
We are all excited for the beginning of another positive school year at Jane Addams! As we Belong Together, Work Together, Succeed Together, we believe in our mission to engage and connect all students through academic and social-emotional learning. Jane Addams is so fortunate to have a talented staff who demonstrate a committment to each other and our students and families. Please let any of us know how we can support you. Attached is our 2023-2024 JA Staff List for you to reference through the school year.
The first day of school is Thursday, August 17th for our students in grades 1-6. Our kindergarten students start one day later, Friday, August 18th.
As a reminder, there is no school Monday, September 4th in observance of Labor Day.
School Hours
It is easy to fall into summer hours. Remember, Jane Addams begins each day at 7:35 am. We offer free breakfast and lunch for all students. Please remind your child to bring a water bottle as we encourage plenty of water through the day. Below are our school hours for your reference.
Office Hours: 6:35 am – 3:05 pm
Grades 1-6 Monday-Thursday 7:35 am – 2:15 pm
Grades 1-6 Friday 7:35 am – 1:25 pm
Kindergarten AM
Monday - Thursday 7:35 am – 10:10 am
Friday 7:35 am – 9:45 am
Registration Reminders
Online student registration for the 2023-24 school year continues to be available. If you have not already started this process for your student(s), please visit our registration page to start this process. As a reminder, students who have not completed their registration requirements will not recieve a classroom or bus assignment. Families who did not complete this process by July 31, 2023 can come to Jane Addams to re-enroll their student.
Register your child. Registration is the first step in getting ready for the upcoming school year. After registering online, parents will need to go in person to your child's school and provide:
Health Forms. Required health, immunization, dental, and eye exams for students entering preschool, kindergarten, sixth-grade, and those who are new to the district.
Prove Residency. If you have a student that is starting Kindergarten, entering 4th grade, or entering 7th grade, you are required to prove residency in District 15. Our residency webpage outlines the documents you will need to provide to prove residency.
Pay student fees. Our student fee webpage outlines the District’s student fee structure and the three ways fees can be paid.

All of the above steps of the enrollment process must be completed prior to the start of the school year. Students of families that, when required, do not prove residency, will not be able to attend class in a District 15 until the family’s residency is verified.
Classroom and Transportation Assignments:
Once fully registered, Classroom assignments will be released in the Campus Parent Portal on Monday, August 7th at approximately 4pm. This is just a few days later than our typical release date due to updated procedures as well as our summer programs running through Thursday, August 3rd this summer.
We have gotten several calls at the Jane Addams Office regarding transportation. Transportation information for the 23-24 school year for eligible students will be available on Friday August 11. Please check back after that time for bus route information. We will also be able to answer questions after that time.
Meet the Teacher Information
We look forward to seeing you on Monday, August 14 2023 from 1:45 - 2:30 pm for Meet the Teacher. At Jane Addams, you will not only meet your teacher for the upcoming year, but greet PTO and learn about a few supports we offer through the year.

Medication Policy
All medication (including over-the-counter medications) requiring administration at school must:
- Be brought to school by the parents and turned into the school office, along with the Medication and Health Care Treatment Authorization Form. This form must be completed prior to medication being given. Medications may not be sent to school with students. All medication must be delivered by a parent, guardian, or other authorized adult to school.
- The medication must be in the original container and have an appropriate label.
- The Medication and Health Care Treatment Authorization Form must be renewed annually if needed in the next school year.
- Any changes in a medication, dose, or time will require a written order from the licensed prescriber.
- The medication must be picked up by a parent/guardian at the end of the year or when the medication is discontinued. Medication that is not picked up will be discarded.
Medication Administration
The administration of medication to a student at school is discouraged unless it is absolutely necessary to allow the student’s academic participation.
- All medication will be stored in a secure location in the health office, with the exception of emergency medications and epinephrine. Students may carry rescue medications within the school properties if additional consent forms are provided and additional training and supervision under the school nurse are provided.
- Medication at school is administered by the school’s registered nurse, a school administrator, or an authorized staff member, in accordance with the Illinois State Board of Education’s recommended guidelines and the Board of Education policy 7:270 Administering Medicines to Students.

Physical, Immunization, Dental, and Eye Exams
For the purpose of safeguarding the health of children, District 15 enforces Illinois student health requirements. These requirements consist of physical (health), dental, and eye exams and immunizations specific to grade and age.
Pre-Kindergarten/ECDEC:
- Physical (Health) Exam form with current immunizations
Kindergarten:
- Physical (Health) Exam form with current immunizations
- Eye Exam form
- Dental Exam form
Second Grade:
- Dental Exam form
Sixth Grade:
- Physical (Health) Exam form with current immunizations
- Dental Exam form
TRANSFER students (from another school in Illinois):
- School health requirements for grade level, as stated above
NEW students (from out of Illinois or country):
- Physical (Health) Exam form with current immunizations
- Eye Exam form
Physical (Health) Exams
PHYSICAL (HEALTH) EXAMS are required for students entering preschool, kindergarten, sixth grade, and those who are new to the school. The Certificate of Child Health Examination form is available online and in the school office.
The Certificate of Child Health Examination form, completed and signed by a licensed healthcare provider, must be dated within one year prior to the first day of school and must include lead screening for children who are six years or younger and diabetes screening. A tuberculosis (TB) skin test is recommended.
According to Board of Education Policy 7:100 Health, Eye, and Dental Examinations, Immunizations, and Exclusion of Students and the Illinois School Code, students who do not provide proof of receiving the required immunization and/or exam are excluded from school on October 15. Students new to Illinois schools, who register midterm, are allowed 30 days following registration to comply with the health requirements.
