Student Informed Consent Form - COVID-19 Saliva Screening
The District has contracted with Safeguard to provide COVID-19 saliva screening to students attending in-person instruction and all interested staff. There is no cost to any family for any screening, this is part of the District 112 COVID-19 emergency funding. We will not be offering this to families and students who are in fully remote learning at this time.

PCR Testing
District 112 is pleased to offer PCR testing through a partnership with Wisconsin Diagnostic Laboratory for all staff and students of 112. The PCR test is an Anterior Nasal Swab and will be completed (with direction from a nurse) by the individual or parent/guardian.
Testing will take place on Mondays and Wednesdays from 8:00-8:30 AM in the front Green Bay parking lot (drive-thru style). Please complete the consent prior to arriving at the testing site. A consent should only be completed when you are coming in for a PCR test. English Student PCR form | Spanish Student PCR form | Staff PCR form. Results will be available within 24-hours. Individuals awaiting PCR results will not be allowed in school until they’ve received a negative result. All of these forms can be found in the Reopening section of the District’s website.
