New Form Required for PCR Testing

The PCR testing company, Ravinia Health, has expanded its own in-house laboratory, Ravinia Clinical Lab (RCL), to better accommodate testing needs. This entails transferring all of its clients from 3rd party labs to its own lab in efforts to provide a more secure and efficient service. Ravinia Health is requiring everyone who is participating in the weekly program to complete a new form. The kit you will receive on March 14 will ask for the following information:

Date of Birth

Sex: M/F

Address

Ethnicity

Phone number

Email

Insurance Information

Signature

Insurance information is required for all participants that have an active insurance plan. This information is required in order for RCL to maintain legal compliance. All COVID-19 testing services are required to obtain this information.

You will not be charged any out of pocket costs. Reimbursement for COVID-19 testing is covered 100% by all insurance payers with no deductible, co-pays, coinsurance, or any other out of pocket expense to the patient. Reimbursement for uninsured individuals for COVID-19 testing is provided by the Health Resources and Services Administration (HRSA).

This transition only requires a one time consent form to be completed. To continue in the testing program, please complete the form next week and return it with the test sample on Tuesday, March 15.

Board of Education Meeting

The next Board of Education meeting will occur at 7:00 p.m. Tuesday, March 15. The notice and agenda can be found on the District 103 Board meeting website page. D103 board meetings are being held in-person and live-streamed each month on our YouTube Channel. Members of the community should attend the meeting at 111 Barclay Boulevard, Lincolnshire if they would like to make a public comment. The public can also contact the Board at board@d103.org.
