Hello Shark Families,

We are entering our last full week of school together for the 2020-2021 school year. While it has been a challenging school year for everyone, I want to thank you for constantly supporting our students and staff at Smith Intermediate and making it a great year. As we wrap up our last 5 days together (and two half-days!) our students will engage in daily leadership activities with the APEX Leadership Team on campus as we continue the last week of our fundraiser. Students will participate in the APEX Fun Run on Friday, May 21st. We look forward to a last week of learning and kicking off summer.

A Note from the Nurse:

I wanted to thank all parents for your support and cooperation this year as we faced an interesting school year full of new challenges. It has been a pleasure taking care of your children. The school year is coming to a close and there are a few things that we need parent assistance with to ensure a smooth transition.


  • All medications must be picked up from the clinic by the last day of school. All prescription medication must be picked up by a parent. Any medication left after the last day of school will be disposed of. If your student has spacers or nebulizer tubing, it will be sent home with your student.

  • Over the counter medications can be sent home with students IF parent permission has been given via email or note

  • Attached you will find the Permission to Administer Medication form for the upcoming school year. These must be filled out every year for medication to be administered. You may print the form as needed and submit it when school resumes in the fall for students who have medication that will need to be administered.

  • Action plans and doctors orders for medications, conditions or procedures must be renewed every school year. Please speak with physicians before school resumes for updated orders/action plans for the upcoming year.

  • If your student has a Waiver of Conscience Affidavit for immunizations that will expire during the summer, they must submit a new affidavit before school begins. Students with expired waivers/vaccinations will not be permitted to attend until a new affidavit/vaccination record is received.

5th Grade- Human Growth and Development (Puberty) Video

5th Grade Parents,

Please see the email sent on the afternoon of May 10th regarding the upcoming video that 5th graders will be watching on Human Growth and Development. The video will be shown on May 20th. If you don't want your student watching the video, please send back the form referenced in the email, or contact our campus nurse.

Covid Guidance for Students

If a student is exhibiting these symptoms, please keep them home and call our nurse.


  • Cough; difficulty breathing or shortness of breath
  • Severe headache
  • Sore throat
  • Loss of taste or smell
  • Diarrhea, vomiting, or abdominal pain
  • Feeling fevering or having fever greater that 100 degree F.


If someone in your household has Covid, or is suspected of having Covid (even if waiting for test results), or your child is exhibiting any of the above symptoms, please fill out the self-reporting form below.


When students are home due to quarantine, they can participate in their school work on Canvas so that they don't get behind. Logging in to Canvas each day and completing assignments counts as their daily attendance.
