Progress Reports
All Quarter 1 Progress Reports should be dated 10/20/2023 and should be sent home no later than 11/01/2023.
Progress Reports should be completed by the student's IEP case manager. Please make sure you are coordinating with the student's entire team (including Related Services staff) to ensure all goal areas are being reported on.
All progress reports should be completed for the IEP that is active on 10/20/2023, even if it was completed recently. If you have no new data collected, please provide the baseline data that you used in the present levels for each goal area.
If the student has a transition plan in their IEP, you must complete TWO separate Progress Reports (one traditional progress report reporting on the student's goals and one transition progress report reporting on the student's transition activity progress).
Mark your calendars for the remaining Progress Report dates (see below):
Quarter 1
Report date: 10/20/2023
Reports must be sent home by: 11/01/2023
Quarter 2
Report date: 01/12/2024
Reports must be sent home by: 01/26/2024
Quarter 3
Report date: 03/15/2024
Reports must be sent home by: 04/08/2024
Quarter 4
Report date: 05/30/2024
Reports must be sent home by (ES/IS): 06/07/2024
Reports must be sent home by (MS/HS): 06/14/2024

ELA Reader Applications for ES Students
SWCSD students will be testing for the Third Grade Reading Guarantee:
October 17th through October 24th
Cathy McCoy has asked that all 3rd grade students who have a reader accommodation on their IEP have an ELA Reader Application completed. This will allow us to track who may need extra support based on scores.
With that in mind, please remember that only some students will qualify for a reader (based on SWCSD guidelines found here).
ELA Reader Applications for 3rd Grade ES Students were be due by:
August 30th
If you have not submitted your student's information, please do so ASAP.

ELA Reader Applications for HS Students
SWCSD high school students will be fall testing for End of Course Exams:
December 4th through December 22nd
Janet Yohman has asked that only the students who will qualify for the ELA reader accommodation (based on SWCSD guidelines found here) have an ELA Reader Application completed.

Completing Progress Reports
- Progress reports should be sent based on the frequency input for each goal area (typically, every 9 weeks). For this reason, almost all reports should be dated the last day of each quarter and should be provided to families the same day report card information is available.
- Parents / Guardians are REQUIRED to receive progress information as part of the IEP team. Progress Reports being sent home in a book bag -OR- sent through email that the parent / guardian will not check is not best practice.
Which IEP should be reported on?
- All Progress Reports should be completed for the most recent IEP that is active on the last day of the quarter.
- If you have no new data collected, please provide the baseline data that you used in the PLOPs for each goal area with an explanation of why no new data has been collected.

- Progress Reports only require data on measurable annual goals (objective data is not required to be included, but should be kept for our documentation) AND transition activities (if student has a transition plan in their IEP).
- There is no specific way you need to present the information from the Progress Report, meaning you can use whatever format you wish as long as it includes all ODE requirements (review detailed guidance slides to ensure you have included all pertinent information).
- There are three recommended versions to use when completing Progress Reports (choose ONE):

- SWCSD Progress Report through Google Documents (view example here)

This can be helpful if you want to upload signature pages or Progress Reports from the Goal Monitoring tab. Remember, in order to upload a document it MUST be in a PDF format.
