Surveys:
Parents and Students:
Please take a few moments of your time to complete this survey. The information is used by the Collins High School Youth Services Center to determine how we may be able to better serve our students and their families. The Collins High School Youth Services Center provides programs and services for ALL Martha Layne Collins High School students and families. This is made possible through funding from the Cabinet for Health and Family Services. To obtain this funding, the center is required to conduct a Needs Assessment Survey to determine programs and how funds will be spent.
This is an anonymous survey. All responses are confidential and questions do not ask for you to identify yourself or your students.
Thank you,
Leesa Stivers

Rake and Run Program:
Every year our BETA club kids do a rake and run event. This event is where the BETA kids go out into the community and rake a yard and then leave before the owners know what is happening. Since this year's event will be held on Veteran's Day, the BETA students would like to say Thank you to our Veteran's by raking their yard. If you know of a veteran that would love this to happen to their yard, please email Leesa Stivers ( Leesa.stivers@shelby.kyschoosl.us) the Veteran's name and their address. Please note we will only share the addresses with the students for this project.
Thank you for helping our students to give back to the community!
