Bi-Weekly Focus: Hematomas
- Ruptured AVM or AN? Brain Bleed? Hematoma?
- Hemorrhage origin: AN / AVM / Unknown?
- Site?
- SAH, ICH, IVH, Hydrocephalus, Other?
- Size? < 3 cm or >3 cm?
- Fisher Grade, if SAH?
- Thin or Thick SAH?
These questions are important when the patient has a bleed. It helps us complete sections on the STAR and SAM form for data collection.
I know that many of you are asked about the ICH score. The ICH Score predicts mortality in patients with intracerebral hemorrhage. Below is a Hemphill Scoring template.
Remember to document these:
- Patient's Baseline Function - Driving? Working? Walking? Independent w/ADLs?
- Handedness: Right, Left, or Both?
- Packing Density: How full did you stuff it?
- Temporary Occlusion Time: How long was it occluded? Complete Occlusion or not? Bypass?
- Flow Diversion Info: Proximal & Distal measurements
