2017-2018 EXEMPTION POLICY
FROM THE CLINIC:
Parents: Please plan to pick up any remaining medication from the clinic by the end of the school day on Wednesday, May 30st . Any left-over medication not picked up will be destroyed. Contacting the clinic with your preference of either pick up or destruction of medication before Wednesday, May 23rd would be greatly appreciated! Our number is: 832-484-5057.
Additional note: Reminder, a new/updated medical plan or pass request from the doctor’s office is required before the first day of school next year. The clinic will open starting in August to happily assist you with this matter.
Graduating students planning to attend college in Texas: Please make sure your Meningitis vaccine has been given within the last 5 years before your college start date. They will check this before letting you sign up for classes and moving in on campus. Most of you will require a new shot as it was required for you to attend 7th grade.
**Attention Seniors! **
Please stop by the clinic to sign up for a copy of your shot record. It is an important document that you will need at varying times in your life, to include college.
After you sign up for it, remember to come back by the following day to pick it up.
*** Due to processing time, kindly submit all requests prior to May 29, 2018.
Thank you!!

More from the Clinic
We are sending out updated shot requests for grades 9,10,11.
If you receive a letter in the mail requesting immunization updates, please provide a receipt of the new immunization to the KOHS clinic asap. Students delinquent with shots or needing updated Immunization Exemption forms will not be able to register for a parking sticker and they will have their schedules held on the first day of class.
Starting next school year all students in grade 9 are required by the state to have the Hepatitis A vaccine which is a series of two shots. Students who will repeat 9th grade are going to need to get this vaccine. The clinic will not know who these individuals are until later in August. If you find yourself in this situation, please be proactive and start the Hepatitis A vaccine series and/or bring the KOHS clinic a copy of your immunization. Thank you for your help in this matter.
As a reminder, all students who have medication in the clinic will need to have their parents pick up the medication by 11:00 am on May 31, 2018, the last day of class.
** Also, remember the clinic requires updated medical documentation at the beginning of each school year for any ongoing daily special needs.
Thank you and have a super summer!
Prom tickets are now $75. They will be sold during B-D lunches. Friday, 5/25 is the last day to purchase!
