COVID 19 Testing Opt-in or Opt-Out Form

San Diego Unified School District continues to work with public health authorities, medical professionals, and scientific experts for a safe, responsible reopening in light of the COVID-19 pandemic. One of the best ways to prevent the spread of COVID-19 is to test for COVID regularly. For this reason, our district is offering free testing for students at school sites. All families are required to submit a Covid-19 Testing Option Form for their student(s), whether they choose to enroll in testing, or not. Opting-in to COVID-19 testing requires filling out additional consent forms. Parents/Guardians and students can use the PowerSchool Parent/Student Portal to submit their Covid19 testing option and consent forms.

Here are the directions on how to complete the form.

COVID 19 Testing Every Monday

For those who have opted in to have your child tested, we will have testing on site every Monday from 12PM-3PM. Please make sure to register them to be tested here. The Opt-in form lets us know who wants to be tested and the registration is for the actual test.
