*You may print and complete forms at your convenience before visiting the school clinic.
**Please remember an adult MUST bring ANY/ALL medications into the school building (including cough drops and sunscreen) with proper paperwork completed before we may administer medications to your child.
SEVERE ALLERGIES:
Severe Allergy Action Plan
*First page to be completed by parent/guardian for students who have an EpiPen ordered for severe allergies; Second page to be completed per physician.
SEVERE ALLERGY SELF-CARRY AUTHORIZATION FORM
*To be completed if your child has permission to carry their own EpiPen.
NON-ANAPHYLACTIC ALLERGY INFORMATION FORM
*To be completed if your child has food, insects, or other environmental allergies that DO NOT require an EpiPen.
ASTHMA:
ASTHMA ACTION PLAN
*To be completed for all students with a diagnosis of ASTHMA. Paperwork must be completed for student to use an inhaler at school at any time throughout the school year.
ASTHMA SELF-CARRY INHALER FORM
*To be completed per parent for any child who may self-carry their inhaler at school, or one who needs to transport it back and forth to school daily in their backpack.
MEDICATIONS:
MEDICATION FORM
*Must be completed for any medications brought into the school building. Please remember your child may NOT carry medications of any kind into the building. Medications must be signed in by an adult.
INSECT REPELLENT, COUGH DROPS OR SUN SCREEN
*Insect repellent may only be brought in WIPES form and applied per student. Sunscreen must be applied per student.
