Hot weather is approaching; please remind your student to bring their water daily. We have water refill stations available. This will help avoid those headaches due to lack of water and help keep your child well-hydrated throughout the day.
Do not send your child to school with symptoms or a diagnosis of a contagious illness. Such as a temperature > 100.4, vomiting, diarrhea, pink eye, strep, flu, and Covid diagnosis & follow up with your physician for guidance. Allergy season is also upon us; please, contact your child's physician for any symptoms of concern.
* All medications must be picked up by Thursday, June 8th, by the parent/guardian before the last day of school. Any medications remaining after 12:00 pm on June 8th will be destroyed per QACPS policy.
QACPS & Md State policy: QACPS students may not attend school unless they have submitted an official immunization record or other appropriate documentation. This immunization record or other documentation must contain the month and year of each vaccination and the signature of a physician or health department official. The building administrator or his designee must approve the submitted documentation.
Please, don't wait until the last minute. ALL 7th graders require proof supplied to the school nurse of one Tdap & and MCV4 vaccine to attend QACPS. Contact your pediatrician for accurate and complete shot records or to make an appointment to receive the required vaccines. This is due BEFORE your child returns to school next year per QACPS & Md State requirements.
Don't hesitate to contact the support services team as needed. Student Support staff includes school nurses, guidance counselors, pupil personnel workers, mental health coordinators, and school psychologists. Thank you for being so helpful in supporting your children's StMS & QACPS health services. Health matters!

Moving this Summer?
Please complete INFOSNAP as early as possible this summer. If you are moving, please follow these guidelines:
- Leaving Stevensville Middle School? ~ please call the school for a Transfer Packet
- If you have moved out of STMS Zone, please call the school in that Zone ASAP
- Please check you are attending the correct school ~ click on this link and type in your home address.
- If parents have joint legal custody of their student, the parent updating INFOSNAP must update INFOSNAP with other parent's information and where necessary provide us with custody documents.
- Moving to a new house within the STMS School Zone, you are required to update your proof of residency by providing STMS with a copy of at least one of the documents requested below. (incorrect information will hold up INFOSNAP approval)
- Proof of Residence (Electric/gas statement, lease (with signatures), mortgage statement, or property tax bill)
- Buying a House, no docs ~ Contract of sale will ONLY be accepted.

School-Based Health/Dental Centers Enrollment Form and Information
As a student in the Queen Anne’s County Public School system (QACPS), your child has access to the Choptank Community Health SCHOOL BASED HEALTH & DENTAL CENTERS at the following QACPS sites: Church Hill Elementary, and Sudlersville Elementary and Middle School.
The mission of the Centers is to improve the health of students and faculty, increase access to primary health care and decrease time lost from school by providing care within the school setting. We are a convenient source of quality health care that works in collaboration with your child’s doctor and the school nurse.
Choptank Community Health recognizes the connection between health and positive academic outcomes. CCHS is pleased to partner with Queen Anne’s County Public Schools and Queen Anne’s County Health Department to ensure that students are healthy and ready to learn.
