Creating a Form on the iPad
- In Google or Safari browser, type in forms.google.com
- Log into your Drive
- student login: username@midwayisd.org
- password: capital last initial, student ID, lowercase first initial
- see your Inst. Tech if you need logins and passwords
- Editable Google form should load in browser.
- Click off Require Midway login for anonymous data collection.
- Create questions
- When done and ready to send form, click box for short url to send from iPad.
- Try creating a QR ode to post around school for easier data collection.

Why Data Collection?
Data Collection and reporting in Project-based Learning or Challenge-based learning is an important step because it gives students something concrete to guide their project and compare results with as they move from the problem to the solution. Data is key in analyzing effectiveness and bring about change.
