Registration for the 2021-2022 School Year
4th Grade Families
We have extended the deadline for registration for the 5th grade instrumental program until Monday, May 17. There has been a little bit of confusion on the process of registering, as it is different from past years due to the pandemic.
All information about the program can be found at www.unit5.org/instrumentexploration.
We have noticed that there are some who are choosing more than one instrument on the signup form. In the past, we have been able to hold an exploration event where students try out different instruments. Unfortunately, we are unable to do that this year. If your student would like to try out different instruments before making a final decision on what to play, we suggest that you head to a music store (The Music Shoppe or Carl’s Pro Band) and they will be glad to help your student find an instrument.
Once you have decided on an instrument, please fill out this form so we can get your student registered for the program next year.

Thank you to Mr. Schultz and Mrs. Malcom for greeting cars, buses and families everyday! The two of them were able to get traffic moving and safe drop offs for all students. We are grateful they were willing to brave the elements and be there for the students. Thanks!
Last Day of School
Nurse Notes
If you have a child that will be starting Kindergarten this fall please make sure you register your future student and provide all medical documents to the school. Your child will need a State of Illinois Physical dated on or after August 18, 2020 and an updated immunization record. Unit 5 has first day exclusion rule, which means if your student does not have a current updated physical and immunization record by the first day of school, they will not be allowed to attend until the school is provided the documents. Vision screening and Dental screening are required for incoming kindergarteners, but they will not be excluded for the two documents if they do not have them by the start of school. If your child has a health condition or will need medication during the school day, you will need to provide the Unit 5 medication form for all medication needed during the school day. We must have all emergency action plans, medication, and medication forms by the first day of school. All forms can be found on the Unit 5 webpage and many of the doctor’s offices have them available at their office.
