As per the Connecticut State Law (C.G.S. Sec. 10-204a and 10-206), all children must provide the school with proof of the following requirements in order to attend 7th grade at the start of the 2021-2022 school year.
• A complete physical exam by a health provider done on or after July 1st, 2020.
• An updated immunization record which includes these vaccines: a 2nd dose of MMR (measles, mumps, rubella), a 2nd dose of varicella (chickenpox) or verification of disease from a health care provider, Tdap booster, and meningococcal.
Please fax this paperwork to (860) 240-6503 or email to firstname.lastname@example.org by August 1st, 2020.
If your child has received medications in school this year, please make arrangements with me to pick up any remaining medication before the end of the school year. I will not be able to send home any medications with the students. Any medications that remain in the office after the last day of school will be destroyed.
If you have any questions please email or call me at (860) 724-0685 ext. 3599
