Steps to Setting Up Google Form:

1. Log in to your Google Acct (Using your Elgin login and password)

2. Choose the grid of apps in the top right corner

3. Choose Drive...second row, middle....Triangle

4. Choose New

5. Choose More at the bottom of the list

6. Choose Forms (purple icon)

7. Name your Form: EHS Teacher Input Form or EMS Teacher Input Form

8. Make sure to check the form at the top for only EISD access

  • Inclusion documentation has begun and information will be shared each Monday. Please report one form per student per class period served

  • Inclusion Walk-Throughs will begin soon. This information will be shared to your administrator each week. You may be asked to provide additional information. Each Walk-through will typically last 20 minutes

  • Keep your case manager lists up to date! Don't forget about 3 week and 6 week updates and AIP/IPI updates once a 6 weeks.
