Important Forms to be Returned to School
- Census Verification Form (on green paper) - this document lists the names, phone numbers, email addresses, and guardian information we currently have on file for your child. Please review the information listed to make sure it is correct. If revisions are needed please make them on the sheet in pen. Sign and return the form. Please note that if there are restrictions on parent or guardian access to your child that the main office must be provided with active, legal documentation regarding the matter.
- KPMS Student Handbook Verification Form (on salmon paper) - all students and parents must review the student handbook, fill-out, and return this form.
- Over the Counter Medication Form (on blue paper) - this optional document allows the nurse to provide certain OTC medication to students. Please fill and out and return the form if you would like to give this permission to our Health Office.
All forms should be returned to the child's Day 1A Period 1 teacher or to the main office.

COVID-19 School Testing Program
COVID-19 Vaccination Clinic
A pop up vaccination clinic will be held on Monday, September 20th from 3-6:30 pm in the bus loop of King Philip Regional High School.
This clinic will provide 2nd doses of the Pfizer vaccine for those students, 12 and up, and any family members who received their 1st dose at the clinic on August 30, 2021. For those interested, 1st dose Pfizer vaccines for ages 12 and up, and one dose J & J for those 18 and up, will be available as well. To register in advance and provide consent for a minor, use this registration link. You may also register onsite.
Family Wellness Resources
Looking for resources to support the wellness of your children and family? The Family Wellness website provides information and resources on a variety of wellness topics, including those for meeting basic needs such as food access, counseling, mental health, staying healthy at home, and more. The events page will include information for wellness education events for parents and caregivers in the regional districts and in other locations. You are encouraged to visit the site regularly. Community Opportunities
Interdisciplinary Artificial Intelligence Initiative by MIT Graduate Students
How can AI help us mitigate the climate crisis, trace the spread of illnesses, build self-driving cars, or identify bias in the criminal justice system? How exactly does Netflix know what your next favorite TV show is before you do? At the AI Scholars program students learn to apply an AI toolkit to tackle such real-world issues in domains they're passionate about– no prior programming experience necessary.
Through a project-based curriculum, students work under the guidance of an AI mentor from Stanford or MIT to explore the ethics and applications of artificial intelligence in healthcare, environment, law, music, astronomy and more! At the end of the course, students gain a sophisticated understanding of AI, experience applying AI to real-world datasets, and a final project they can use to enhance their college portfolio and careers.
