Message From Our Athletic Director, Mr. Cunningham
This e-mail contains important information for students interested in participating in sports at Newtown Middle School in the 2020-2021 school year. In order to participate or attend tryouts, every student must have a completed PIAA athletic physical packet, an online CCET concussion-education program, an online Impact baseline concussion test and a COVID waiver. All four items must be completed by Friday, September 11th, 2020 for all FALL athletes. Our Fall Athletic Programs are:
- Lightweight Football (tryout start 9/14)
- Heavyweight Football (tryout start 9/14)
- Boys Soccer (tryout start 9/14)
- Girls Field Hockey (tryout start 9/14)
- Girls Volleyball (tryout start 9/14)
- Cheer (tryout start 9/21)

#1: PIAA Athletic Physical:
We now use ATS, the on-line software program that allows students, and parents to log in and upload all the forms. Therefore, we are not accepting paper copies of the PIAA athletic physical packet. Please do not hand in or email any paperwork into a coach, teacher, counselor, or nurse. Please also do not mail any paperwork to the school. These will all not be accepted.

This is the student login that will take you to the student’s account. Once you are in the system, you can change your password. Please note, if you are an 8th grade student your password from last year will no longer be valid this school year. Please use the default password listed above for the first time you log into the account.
If you see information that is incorrect or out of date in ATS, DO NOT attempt to correct it in the ATS system.
To correct information please log in to the HAC system (https://hac.crsd.org/HomeAccess) and make updates there or contact Central Registration (Registration@crsd.org) to make updates.
To fill out the physical packet click on the “Forms” tab. There are 5 forms that need to be completed.

PIAA SECTION 2: CERTIFICATION OF PARENT/GUARDIAN

FORM 6: Under the “eFiles” tab you will find PIAA Section 6. The PIAA and CRSD requires a current, valid physical that is dated by a medical provider after June 1st, 2020 in order to participate in any athletic activity. We DO NOT ACCEPT any other Physical Form other than the Section 6 – PIAA CIPPE Form that is located under the “efiles” tab. Please print out the form and take it to your physician. You can scan the completed PIAA section 6 form (or take a picture with your phone) onto the ATS website.
Please do not include immunizations. These papers MUST go to the nurse’s office directly.
Please note, once you upload PIAA section 6 it will show up on the uploaded list but will not show as checked off under paperwork until someone visually verifies the page.
#2: Online CCET Concussion Education Program:
Due to the COVID-19 pandemic, concussion education and ImPACT testing will be completed online from home. The CCET Concussion Education Program is an informational presentation designed to educate students on concussion prevention, symptoms and follow-up care. Each section will have questions that students will have to answer in order to complete the program. When you have completed the presentation and all the questions, it will show you your score. Your Athletic Director will be able to see who has completed this presentation and the score you receive on the questions.
*This education program should be completed by the athlete before taking the ImPACT Concussion Baseline Test.

#3: ImPACT Concussion Baseline Testing:
The ImPACT Test consists of several cognitive questions, which will provide a baseline for future impact testing in the event the student suffers a concussion during the Holland Middle School athletic season. The baseline can then be referenced to help determine an athlete’s readiness to return to practice and competitions. Concussion testing is MANDATORY for every student athlete except for Track.
*If you are an 8th grade student who took the concussion test last year, you will still need to take it again this school year. Concussion tests are valid for one school year.

#4: Covid Waiver: You must turn in the COVID waiver signed by a parent and the athlete. This is handed in at the first day of tryouts to your coach.

Directions - Online CCET Concussion Education Program
1. Click on the link:
https://chpswtemple.co1.qualtrics.com/jfe/form/SV_6RtZWOjWTytrAbP to open the online learning module for Concussion Education. It will say Temple University College of Public Health
2. Fill out your FIRST and LAST name. Please do NOT use nicknames. Then click on the arrow to advance to the next section.
3. Select the SCHOOL BUILDING (Newtown MS, CR) you will be attending in the fall. Click on the arrow.
4. In the next section put your Council Rock School EMAIL ADDRESS, then click on the arrow.
5. Select what GRADE you will be going into this FALL 2020, then click on the arrow.
6. Select any SPORT you will or plan to play in the 2020-2021 school year, then click on the arrow.
7. The next section is the PRESENTATION. If it does not initially show up, click on the screen to START the presentation or press the PLAY button.
8. Each section of this presentation takes approximately 7-10 minutes.
9. You may have to view it in full screen.
10. At the end of each section of the presentation you will have to click on the arrow at the BOTTOM RIGHT of the screen.
11. When the presentation for each section is completed you will need to answer the MULTIPLE CHOICE OR TRUE/FALSE questions in each section.
12. When you have completed the presentation and all the questions it will show you your score.
13. Your Athletic Director will be able to see who has completed this presentation and the score you receive on the questions.
