Health Survey

The health survey below was emailed to families of students attending hybrid learning. Please take a few minutes to complete this survey if you have not already completed it.

Hybrid Student Health Information

WEARING A MASK

Students and staff are required to wear a mask at all times during the school day and on the school bus. We are asking that students bring an extra mask with them daily. Please put the extra mask in a Ziploc bag in your student's book bag.

SAFETY MITIGATIONS

We will have the following safety mitigations on a daily basis:

  • We will take student temperatures when entering the building each day.

  • Masks must be worn properly at all times. Refusal to wear a mask covering the nose and mouth may result in a switch to remote learning for students.

  • Frequent hand washing and/or use of hand sanitizer when changing classes.

  • Students will wipe off their desks each class period.

  • Maintaining distance in the hallways and in the classrooms.

  • Students will not share supplies or materials.

WRISTBANDS

Wristbands can be picked up in the main office at James Hart when returning the

Daily Health Certification Acknowledgement Form through Monday, March 1st. The office is open from 7:30 am-3:15 pm.

SCHOOL COMPUTERS

Students will need their district-issued school laptops daily. Students will still use their devices to access instruction and submit assignments.

School laptops must be fully charged each day. There will be limited opportunities to charge devices during the school day.

Students are responsible for the care of their laptops and should use the carrying case provided by the district. Laptops should be restarted each day to make sure they are updated.
