TITLE 1 SCREENING OCTOBER 30- NOVEMBER 11TH
It is time once again for Title I screening. The Title I Screening Team will be screening children at your child's school next week. At this time, we will conduct a school-wide health screening with emphasis in the following areas:
- Medical/Dental/Vision/Height/Weight evaluations on all children in Child Development
- Medical/Dental/Hearing/Vision/Height/Weight evaluations on all children in grades K-6
- Evaluations according to needs on all children in Special Education classes
Under Title I guidelines, it is required that any noted concern be referred back to the parents. If a concern in one or more of these areas is found, you will be sent a packet of referral forms. Please read, sign the forms where indicated, and return them to the school as soon as possible. This information will be recorded on your child's permanent health record to help your child as needed. We realize you may already know of this area of concern and be in the process of seeing a doctor/dentist/eye doctor for this condition; however it is our responsibility as a health screening team to inform you of any concern we find. If you have prior knowledge of this concern and are in the process of having it treated, please just write this on the parent acknowledgement form that you return to school.
Our goal is to help each parent be as informed as possible and to get and keep each student at an A+ level of health. Your cooperation and timely response to our referrals will help us make Florence School District One a healthy and informed school district that places the health of our children first. Remember, Healthy Children Learn Better!
Thank you,
Title I School Nurses
