Vision Screenings for K, 1st, and 3rd
Grade K and 3 Teachers
· This will be a very quick acuity screening. Each K and 3 teacher will receive a basic class roster from the office. You’ll line your students up into alpha order and bring them into a screening room and give your roster to the doctor. The doctor will mark pass or (DNP) did not pass next to each student’s name.
· You’ll copy your roster after the screenings and give one copy to the office. They will pass this along to the nurse.
· You’ll keep a copy and then call home on any DNPs. We ask that you inform families that their child was screened and some possible vision concerns were noted. You’ll let them know that you’ll be sending home a referral sheet. Families should take this referral sheet to their family eye care provider to see if any further action concerning their child’s vision is needed.
· We truly appreciate your efforts in all of this and know that our families appreciate the personal contact.
· Please ask families to keep you up to date on any developments from their eye care provider. Please also FW those updates onto your nurse. They will keep notes on student needs concerning glasses, etc.
Grade 1 Teachers
· Your students will participate in a more thorough screening now required at grade 1. It will be quick but will require visiting a few stations in the screening room.
· Each grade 1 teacher will receive a basic class roster from the office. You’ll also receive a MCT sheet for each child. You’ll put your student’s name on the top of this MCT sheet.
· You’ll line your students up into alpha order, ask them to hold onto their MCT sheet and then bring them into a screening room. The doctors will mark their sheet as they travel around quickly to each station.
· At the end of their stations you’ll head out and collect all of their sheets. Please make a copy of these to keep as a class set.
· Then use your class roster to mark pass or (DNP) did not pass next to each student’s name. The doctor will indicate Pass or DNP on the MCT sheet.
· You’ll then copy your roster after filling it in and give one copy to the office. You’ll also give the office your class set of copied MCT sheets. They will pass these along to the nurse.
· You’ll keep a copy of your roster and then call home on any DNPs. We ask that you inform families that their child was screened and some possible vision concerns were noted. You’ll let them know that you’ll be sending home their child’s MCT sheet which contains information for their family eye care provider. Families should take this MCT sheet to their provider to see if any further action concerning their child’s vision is needed.
· You can also send home the MCT sheets for students that passed.
· We truly appreciate your efforts in all of this and know that our families appreciate the personal contact.
· Please ask families to keep you up to date on any developments from their eye care provider. Please also FW those updates onto your nurse. They will keep notes on student needs concerning glasses, etc.
