Youth Risk Behavior Survey
During the week of November 30th, Forest Park Middle School will take part in the Youth Risk Behavior Survey sponsored by the Department of Public Instruction. Students will be asked to fill out an anonymous, online questionnaire that takes about 25-35 minutes to complete. They will take the survey during regular class time. The results of this survey help schools and the community to improve health and safety programs. We would like all selected students to take part in the survey, but the survey is voluntary. We will be sending out additional information regarding the survey via school messenger which will also contain information about how to opt your child out of the survey.

Will Your Student Be Attending The FHS Football Game In Madison This Friday?
If your student will be leaving early on Friday with another student and their parent/guardian, you must indicate who they are leaving with on the absence form. We cannot release students to anyone other than their parent/guardian or emergency contact unless the parent/guardian has communicated who will be picking them up ahead of time.

FHS Senior Abby Martin Returns to Food Network
Many families may remember when former Pleasant View and Forest Park student Abby Martin competed on Food Network’s Kids Baking Championship Season 4 in 2018. If you missed Abby in Season 4, the series is still available to watch on Food Network or Hulu. Abby is currently a senior at Franklin High School, and is excited to announce that she has been invited back to Food Network!
This time Abby will be co-hosting a Kids Baking Championship Holiday Special with Duff Goldman. “Light Up the Holidays” will air on Sunday, November 21st at 7:00 pm CST. The holiday special will feature four fan-favorite kid bakers competing in a new challenge. Please share the news and tune in to Food Network next Sunday night to see Abby in her new role as co-host and judge!

It's Getting Cold Out There!
Just a reminder to please be sure that your child is dressed for the weather. All students will continue to go outside for recess even as the weather gets colder. The decision to have an outdoor or indoor recess is based on both the wind speed and temperature. Students do not go outdoors if it is raining or if the temperature falls below 0 degrees wind chill. Please encourage your students to dress appropriately as they will continue to go outside for recess.
Forest Park Athletics and Activities UPDATE
Please refer to our website to learn more about Forest Park’s sport and club offerings. All registrations for sports/clubs must be done online and all have a deadline to join. No paper forms will be accepted in the office. Each sport/club charges a $25 fee and this shouldn’t be paid until AFTER a sport/club begins. Fees should be paid online in PowerSchool once they appear in your student's account.

Youth Musical: Annie Jr!
Leapin' Lizards! Forte Theatre Company announces auditions for their 2021-2022 youth musical, Annie Jr! Students ages 10-18 are invited to sign up for an audition slot on December 3rd or 4th. Food Service Reminders
New Lunch Procedures
In the last couple weeks, in an effort to move the Students through the Lunch Lines quicker, we implemented a New Lunch Procedure. We have designated 2 Lunch Lines for Student ID Scanning and 1 Lunch Line for Manual Student Number Entry. Students who bring their Student ID to Lunch can use one of the two lines designated for Scanning and Students who do not bring their Student ID to Lunch have to use the Manual Entry Line.
We strongly encourage Students to bring their ID everyday to make their lunch line wait shorter, allowing them more time to eat and giving them more time with their friends. Additionally, please remind your child to enter the correct line because any student entering the Scanning Line without an ID will be sent to the Manual Entry Line making their wait even longer.
Snack Line
On Monday, October 18th, barring no new setbacks, we will be opening our Snack Line. Our Snack Line will have a variety of snacks from Hot Fresh Cookies, Chips, Yogurt, Water and Juice options. Please understand that right now supplies are limited and there may be days where there are very few choices. We will offer what we have and do our best to have as many options as possible. To make a purchase in the Snack Line there are 2 requirements:
#1 You must have your ID to make a purchase
#2 Your Child must have money in their account.
The Snack Line will be open daily providing we have the supplies and staff to keep it open.
Milk-Only Line
Many Students bring cold lunch from home and would like to purchase milk to go along with it. In an effort to make it easier for those students to purchase milk only we are opening a line right next to the Snack Line to purchase milk only! We are hoping this will make it easier for all students who want just milk.
We are trying to make your child's lunch experience as easy as possible and encourage you to remind your child that kindness and patience goes a long way.
Absence Notification Procedures
Like last year, we will be asking parents/guardians to use THIS FORM to report student absences. The attendance form is also located as a button on the main school webpage and under the “Parents Information” section.
This form will require you to enter your child’s six-digit student ID number and birthdate before you can report your child absent. Student specific medical information collected by the form will remain confidential and only be shared within the health and nursing services team.
Visitors & Volunteers in Schools
In regards to visitors or volunteers, any person entering any school building during the school day when children are present for school is required to wear a mask, regardless of vaccination status. Masks should be worn at all times when in the building, and the visitor/volunteer should limit direct contact with others and maintain social distance (6 feet or more) when able.
Visitors and volunteers who are outdoors during the school day where students are present are required to wear a mask unless they have been vaccinated. Visitors or volunteers in the building after school, in locations where students are not present, are not required to wear a mask.
Early Dismissals & Late Arrivals
If you come to pick up your student during the school day, we ask that you do not enter our building but instead pull up into the circle driveway outside of our main office. Your student will be watching for you and we will send them out to your car when they see that you have arrived. If you are waiting longer than a few minutes for your student, you may call our main office to check in (414-529-8250).
If you are dropping your student off late for any reason, you do not need to come into the school with your student. Please pull into our circle driveway outside of the main office where your student can enter and check in. We will give your student a pass to class and update their attendance at that time.
Please remember to use THIS FORM for ANY attendance related notification including students leaving early, arriving late or full day absences. Please do not send notes with your student to notify us of these changes.
Dropping something off for your student?
In an effort to reduce the amount of interruptions to students and teachers in the classroom, most items will be delivered to students during their 3rd hour class unless the item is clearly needed right away (for example, the student's Chromebook). If an item is dropped off after 3rd hour, we will get the item to the student as soon as possible.
