Initiation Report Form
Standards & Expectations Self-Assessment
It is suggested that organization executive boards work together to complete this form, which will be somewhat time-consuming. However, only one user may submit the form (typically the chapter/colony president). This form has been open and accessible since August, and does have the ability to save your progress and return later before the final submit.
Organizations will receive a performance evaluation based on their submission with their Fall 2018 grade report.
Mid-Year Housing Exemption Applications
Students interested in this opportunity should submit the 'Sorority & Fraternity Life Mid-Year Housing Exemption Request' at www.ohio.edu/myhousing between November 19 and December 19. In order to qualify for an exemption, students must be on the roster for their organization, be elected/appointed to the role of President/Treasurer/House Manager, have an overall accumulative GPA of 2.500 or higher, and be in good standing with the University.
Students who meet these requirements and are recommended for approval will be notified between January 2 - January 4 with a confirmation that their contract has been cancelled and instructions on how to move out on January 12 or January 13. Students who do not meet these requirements will receive a denial message by December 21, with instructions on how to appeal the decision. Appeals should be submitted by January 2 for consideration.

Final Fall 2018 All Council Meeting
As a reminder, it is an expectation of the Standards & Expectations process that 1-2 representatives from each chapter/colony attend these meetings.
