Student Check Out '19-20'

Thank you to those of you who have completed this form. If you have not please do so now.


Student Check Out Form- Please take time to fill out the form below. It will help guide us as a band staff regarding instrument check-in and closing out the year information. If you have any questions please let me know. We are asking that every student/family in the program fill out this form by Monday 5/11 as it impacts our district mandated instrument collection day on Monday 5/18. Thank you.
