A few reminders:
All medications kept at school must be picked up by Wednesday, May 23rd
If your student will keep medications at school next year, a new Parent Provider Authorization form is needed.
Please bring medications and PPA forms to school prior to the first day of school. I will begin accepting medications August 2nd.
Parents/guardians of rising 6th grade students: don’t forget the Tdap vaccine requirement!
