D103 Offers Vaccination Clinic for Ages 12 and Older
Dear D103 Families and Staff,
D103 is pleased to announce that a limited availability of the Pfizer-BioNTech vaccine will be available during a COVID-19 Student/Family/Staff Vaccination Clinic on Sunday, May 23, at Daniel Wright Junior High School.
On Monday, May 10, the Food and Drug Administration issued an Emergency Use Authorization to Pfizer-BioNTech, which authorizes use of the vaccine for children ages 12-15. The CDC states that the vaccines are safe and effective and recommend getting vaccinated as soon as possible. Please review this information from the Lake County Health Department regarding vaccines for children 12-years-old and above.
Through a partnership with the Doctors Test Centers, D103 has secured a limited number of first doses of the Pfizer-BioNTech vaccine that will be available to our D103 students over the age of 12, family members, and staff members’ children over the age of 12.
The vaccine will be delivered in two doses. Vaccine appointments for the first dose of the Pfizer-BioNTech vaccine will take place between 10:00 a.m. and 4:00 p.m. on Sunday, May 23. The second dose will be administered on Sunday, June 13, and appointments will be scheduled after the vaccine is administered on May 23.
Important notes regarding these appointments:
Vaccinations are by appointment only. There are no walk-up appointments available.
A parent, guardian, or someone over 18 years old (with a note from the parent who can be reached via phone) must accompany the registrant.
You can receive a vaccine for free even if you lack a photo ID for health insurance. However, if you are insured, please bring a copy of your insurance card.
Second doses will be scheduled for Sunday, June 13 at Daniel Wright Jr. High. Registrants must be available for both dates.
Appointments will be made available on a first-come, first-served basis. Approximately 500 doses will be available.
Appointments may be scheduled online using the self-registration portal at https://tinyurl.com/VAX103. If you are scheduling more than one appointment, please be sure to click the link for each appointment.
Once you register, you will receive an appointment confirmation email. Please complete your medical intake paperwork prior to your appointment to help ensure there are no delays when you arrive for your appointment.
At Your Appointment:
Plan to arrive at Daniel Wright Junior High ten minutes prior to your appointment. If you arrive earlier, please remain in your vehicle until ten minutes before your appointment.
Bring your completed authorization documents and your insurance card (if you have one) with you.
Parent and/or Guardian should bring their ID confirming your D103 residence or a D103 staff ID badge.
You will not have to pay for the vaccine.
After you receive your vaccination, you will need to wait a minimum of 15 minutes for observation.
If you are unable to make an appointment at Daniel Wright Junior High, the Lake County Health Department has space to accommodate your child and family population. Appointments can be made at the Lake County Fairgrounds through the AllVax system, or at the Round Lake site through Curative.
