CHICKASAW PRIDE
YOU are now a CHICKASAW with Chickasaw FRIENDS, and are part of our Chickasaw COMMUNITY. We have Chickasaw RULES, and a Chickasaw WAY of doing things. So when it comes time to make a DECISION, ask yourself, "What would a Chickasaw do?" So put on your best Chickasaw SMILE, because every day is a GREAT day at BES to be a CHICKASAW!

Notes from the Nurse
1. If you have insurance through the school and are interested in participating in the catapult health clinic on Tuesday the 18th and have not signed up please use this link http://www.timeconfirm.com/ARBenefits to do so. Many of you have already registered and this is not a required screening.
2. If you are interested in receiving a hepatitis A and or tetanus shot please have your form filled out with a copy of your insurance card. Some staff members have emailed me a picture of their insurance card and I have printed them off and attached them with your forms. However if you wish to get your information to me please make sure you do so by Monday, Sept 17th, afternoon. As a reminder this clinic will be held September 25th on our campus from 11:20-12:20.
3. STAFF FLU FORMS: In your mailbox in the lounge there are forms for our flu shot clinic that will be in the library on Oct 9th from 9-12. Also you will find a flu vaccine information sheet for you to read in case you might have questions or concerns. You are not required to receive a vaccine! This is a service that is provided by the local health department and does not require a copy of your insurance card to be attached. Please fill out the form to the best of your ability if you wish to participate. Please try your best to have these turned in to me by Oct 8th so the process can go smoothly. However if you cannot get them to me by the 8th you are more than welcome to fill out the form on site the day of the flu shot clinic.
4. STUDENT FLU FORMS will be going out next week. Please hand these out to your students. I will be placing 30 copies in a folder for each class. As students bring them back place them in the folder so I can keep them organized by classroom. I will pick up all folders a week before the clinic on October 9th. If you have students who want to turn in forms after I have picked up your folder please send them to the nurse. STUDENTS WHO DO NOT HAVE THEIR FORMS TURNED IN BEFORE OCT 9TH CANNOT PARTICIPATE WITHOUT A PARENT PRESENT TO FILL OUT CONSENT. We will not be calling parents the day of the clinic to ask them if it's okay for students to participate or to come fill out the forms.
Thank you all so much for your cooperation! I deeply appreciate all of your efforts in keeping our campus and our students healthy. If you have any questions or concerns please feel free to let me know.
Special Dietary Needs
If a student has special dietary or medical needs please contact the school nurse. All students who wish to take medication at school (including inhalers) must receive a form from the school nurse to be filled out by their doctor.
