utah school climate survey
Dear Parents/Guardians:
Please read all the information below as it pertains to both the parent/guardian survey, as well as the student survey.
We want to hear about your experience with your student’s education. We invite you to participate in completing the Utah School Climate Survey. This survey includes questions about your student’s learning, school safety, and interpersonal relationships, as well as your attitudes about the institutional environment and your personal involvement in school. Your responses will be very helpful in improving student relationships, learning conditions, and the overall school environment.
We invite all parents, guardians and caretakers to complete this survey for our school. The survey is completely anonymous, and takes on average 10-15 minutes. You can take the survey online by using the following unique school link: SFJH School Climate Survey. Please submit your responses by May 5th.
There are no right or wrong answers and we have no way to connect you to your responses, which are all reported as group responses. These Responses are housed securely in an anonymous format with The Utah State Board of Education (USBE) for evaluation research purposes. All evaluation research projects are in compliance with the Family Education Rights and Privacy Act, (343 CFR 99.31 (6)) and human subjects regulations (Protection of Human Subjects 45 CFR 46).
As you respond to each item, think about your own personal experiences as a parent, caregiver or guardian at your student’s school. Please answer all of the questions. When you have completed the survey, click on the “Next” button at the bottom of the page, and you are done!
We hope you will take the time to provide us with this valuable information. Feel free to call our office at (801) 792-4075 if you have any questions. Each school’s total survey results will be used to inform and develop appropriate strategies, procedures, and/or programs to enhance our school climate.
Students will be completing the survey on Tuesday, April 11th, following Spring Break. If you would prefer to opt your student(s) out of participating in the survey, please pick up an "Opt Out Form" in the front office and return it no later than March 31st.
If you are interested in viewing the student survey, feel free to come by the front office anytime to view a copy of the survey questions.
Again, here is the link to complete the parent/guardian survey: SFJH School Climate Survey
Thank you for your time and participation!

SHARP Survey
Every two years, the state and district conduct a survey regarding student health and risk behaviors. Information gained from the survey will allow your school district and the State of Utah to continue to provide comprehensive prevention programs for our schools and children. This survey is optional and anonymous and will be offered in their Science classes. Since this survey includes questions regarding risk taking behavior, including drug use, violent behaviors and other personal issues that are not typically discussed in school; parent permission is required for students to participate.
Only students with parent permission will be surveyed. To ensure that you have received this information, please fill out the permission forms linked below. If you do not want your student participating in this survey, please indicate so on the permission form. More detailed information on the surveys are given in the permission forms and below.
