HDC Form in Parent Portal
The Handbook and Media forms are super easy jotforms for you to submit, but the HDC form must be completed in the Parent portal. You should've received your Parent Portal login information in a separate email. Please complete the Household Data Collection form for each of your registered students in Parent Portal.
Please be as honest as possible with the information.
If you wish to NOT disclose your income, you will need to add "0" for both the income and household.
This is to be completed by this Friday, August 25.
Star360
- All TK/K students are to take the Early Literacy test only.
- **New this year- 1st grade students will take the Early literacy and Math tests, NOT Reading
- All 1st-12th grade students are to take the Reading AND Math test.
Just a reminder that failure to complete the test within the testing window will result in Step 1 of non-compliance. I would recommend NOT waiting until Sept. 1- we have run into issues in the past with the system being overwhelmed with all the last minute log-ins. I truly appreciate your help completing this in a timely manner!
LP Meetings
*There were a couple changes regarding 1st and 2nd Tuesdays/Wednesdays of each month, depending on holidays/vacations. For my 1st Wednesday families, for October and May, we will meet on Wednesday of the first full week, as opposed to the actual 1st Wednesday.
*We will not have an LP meeting in December. Our LP4 meetings will take place in January after Christmas break.
Work Samples
Our first work sample will be Math. Even though our LP1 meetings will be in September, this work sample is due no later than Friday, September 1. Texting me a screenshot or emailing a pdf, whatever's easiest for you is totally fine. Please make sure the following are included on the work sample:
- Student first and last name (they must write their own name- for TK/K, you may write their last name)
- Date the assignment was completed (please make sure it's a school day- Saturdays, Sundays and any holidays won't be accepted)
- Subject (Math this time)
- A grade from you (TK-3rd grade can be a star or happy face, 4th-12th grade must be a percentage, letter grade, or ratio)
Please keep in mind that work samples should reflect the student's best work.
