Mrs. Gretchen Hayes Memorial Scholarship
The Hayes family will be offering a yearly scholarship in honor of their mother and grandmother to a graduating senior of Forest City Regional High School. The scholarship will be awarded yearly to a student who approaches life with optimism, a love of learning, and a desire to make the world a better place; all qualities Mrs. Hayes shared with the students of Forest City Regional during her time here and students around the country during her over 40 years in education.
The amount of the scholarship may vary from $1,000 to $2,000 based on the number of qualified applicants awarded the scholarship and the financial condition of the scholarship fund.

“Rotary International is an international service organization whose stated purpose is to bring together business and professional leaders in order to provide humanitarian services, encourage high ethical standards in all vocations, and help build goodwill and peace in the world.”
The goal of this local scholarship: The graduating senior should have demonstrated ambition and self-drive as evidenced by community service or work-related activities. The student will have excelled in volunteering/service with the goal of helping others.
Requirements:
1. Fill out the information on the back of this sheet.
2. The student should be in good academic standing and provide official high school transcript.
3. One letter of recommendation from a teacher, employer, or community leader.
4. Letter of acceptance from attending college
5. The winner will provide proof of commitment to college (e.g. deposit info)
6. Write an essay (500 words maximum) describing their community service, why they chose the project/service, and how the community service has impacted others and their own lives.
7. Applications must be handed in to the guidance office by deadline: Friday, May 19, 2023.
Erin Jessica Moreken Scholarship
This $1000 scholarship was established by the Erin Jessica Moreken Drug & Alcohol Treatment Fund, Inc. in 2014. It was created to assist a graduating senior who has a financial need and is scheduled to graduate in June of the current school year. Criteria for eligibility has been determined as follows:
1. Candidate must be a High School Senior from a local high school.
2. Candidate must have all requirements necessary to graduate in June, 2023.
3. Candidate and/or immediate family member have or have had drug/alcohol issues (also known as Substance Use Disorder), and the candidate has succeeded despite this dysfunction within the family. Candidates who themselves are in recovery must have at least one year sobriety, have a sponsor, and be a member of a support program.
4. Candidate must have been accepted into a post high school program, with plans to further his/her education/career before funds are disbursed. Post high school acceptance letter must accompany application.
5. Scholarship may be awarded anonymously if requested by recipient.
Joseph Nebzydoski Scholarship
2023 Joseph Nebzydoski Scholarship
Applicant Criteria: A graduating senior of Forest City Regional School District
A senior student that has donated a minimum of 30 hours of voluntary community service to either his/her school, fire department, ambulance, church, Boy Scouts, Girl Scouts, 4-H, etc. or other non-profit agency outside of the school day and NOT part of another project (graduation project, Eagle Scout project, 4-H project, etc.).
It is recommended that the scholarship applicant be enrolled in a college or trade school after graduation, but it is not a requirement. The applicant should have an idea of their contributing role to society after graduation.
In addition to completing the attached application, the applicant should attach an official high school transcript, a copy of his/her high school attendance record, a letter of reference from someone in authority at the non-profit agency attesting to his/her hours of volunteerism, and another letter from someone benefiting from his/her acts of volunteerism.

The Jerry Robling Scholarship
History:
- Jerry Robling was a lifelong resident of Scranton, Pennsylvania, and spent most summers of his adult life at beautiful Lake Winola in Wyoming County. As a youth, Jerry rarely left his South Scranton neighborhood, having attended and graduated from St. Mary of the Assumption Grade and High Schools. That changed dramatically when Jerry joined the Army, and then spent the better part of five years with an anti-aircraft unit in places like North Africa, Italy, and at the end of the War, in Germany. After the War, he completed his college education, earning a degree in accounting from the University of Scranton. He then worked as an Internal Revenue Service auditor for over 30 years.
- Jerry and his wife, Marian, who was a teacher for almost forty years, knew the value of getting an education. He also knew that getting a college education had become increasingly difficult as the costs of higher education grew wildly, outpacing the cost of living by a significant amount. Jerry wanted to help kids who grew up like he did to achieve a college education without incurring massive amounts of debt. Consequently, in his Will he established The Jerry Robling Scholarship.
- The Scholarship will be awarded to applicants who live in either Lackawanna or Wyoming County, regardless of where they attend school. The recipients award will be for their first year of school and will not be renewable. The scholarship fund is managed by the Trustees Jerry chose, and those individuals will also be responsible for selecting the scholarship recipients. The IRS Form 1023 which was filed with the Internal Revenue Service to create the Private Foundation which oversees the scholarship is available for review upon request.
Deadline:
To be considered for a scholarship, applicants must submit their application and all supporting material on or before May 15, 2023. The completed application should be submitted to the

Interview:
- Scholarship finalists, as determined by the Scholarship Committee, may be required to participate in an interview process with one or more members of the Committee.
In the event a scholarship recipient fails to matriculate at a school listed in the application within nine months of receiving the award, the scholarship shall be deemed to have been withdrawn, and may be given to a different applicant. Provided, however, if a scholarship recipient decides to attend a school other than that listed on the application, the Trustees may extend the offer.
Grant Process:
The Trustees will notify applicants to whom scholarships will be awarded by July 1, 2023. Such notice will include the amount awarded and directions to provide the Trustees with the information required by the Trustees to make the scholarship payment directly to the college, university, or technical school which the recipient will be attending.

Harford Agricultural Society Scholarship Program
1. Be a High School Graduate
2. Be currently admitted to a school of advanced education, i.e. college or university

2023 Thomas J. Butler Memorial Scholarship
To be considered, this application must be received no later than May 1, 2023.
2023 Order of the Eastern Star (OES) Scholarship
Laurel Chapter, Order of the Eastern Star, awards three $500 scholarships to high school seniors in the local communities who exemplify good citizenship and high moral character; one to a student at Forest City, one to a student at Wayne Highlands, and one to a student at Western Wayne.
SELECTION GUIDELINES
The recipients are selected by Laurel Chapter Scholarship Committee. The following guidelines are used in the selection of recipients:
1. Each Eastern Star Scholarship will be awarded to a high school senior who exemplifies good citizenship and high moral character.
2. While scholastic standing and parental finances may be considered, neither are the sole criteria in the selection of a recipient.
3. It is not the intent of Laurel Chapter that these scholarships be awarded exclusively or primarily to relatives of Eastern Star members or to members of Masonic youth organizations (Rainbow for Girls, DeMolay, Job’s Daughters). These scholarships are available to all youth in the community.
4. The high school may, at its discretion, encourage qualifying students to submit an application.
DUE DATE
The applications should be submitted to the guidance office by April 24th This will give Laurel Chapter Scholarship Committee sufficient time to obtain the applications, review them, and select recipients.
David M. Cavalieri Memorial Scholarship
The scholarship is being offered to a Forest City Regional senior students that intend to pursue post secondary education. Potential recipients need to be active athletes in at least two (2) varsity sports. They must be in good academic standing and be able to show evidence of being compassionate and kind to others.
Scholarship(s) of one thousand ($1,000) will be presented annually at Forest City Regional High School's Class Night. One or more scholarships will be awarded each year based upon the eligibility of the applicants. Applicants must complete the information attached below and submit this form with the additional requirements to the Mr.Daniel Nebzydoski by May 1st of the school year.
Dave Weaver Scholarship
1) Any parent(s) owning, working for or working in a flagstone, fieldstone, or bluestone quarry, processing plant, or distribution yard, including office personnel the truck drivers, with a graduating son(s) or daughter(s) who continue their education immediately after high school.
2) Regardless of your grade point average you can apply.
3) There is no preference in regard to the institution that the applicant plans on attending after graduation, meaning it can be a tech school, beauty school, trade school, college or university.
4) Applicants do not have to be members of the Pennsylvania Bluestone Association to apply for the scholarship but it will affect your overall score. PBA members receive 10% while those working for a company that is a PBA member will receive 5%, and those not members receive 0% for this category.
SAM ROWE III MEMORIAL SCHOLARSHIP
Two $500.00 scholarships, sponsored by Sam’s family, will be awarded in memory of Sam Rowe III to students who intend to pursue a career in a medical field. The scholarships are intended for students who have a commitment to a medical career and will not be awarded solely on academic record. The application below and the required attachments are to be submitted to the Counseling Office no later than Friday, May 12, 2023. All applications with attachments will be forwarded to a committee selected by the sponsors. The decision of the committee is final.
Andrew Mazza Diesel Jam Scholarship
The Andrew Mazza Foundation started in 2016, after the sudden loss of Andrew at the age of 23. Andrew was a passenger in a car that had flipped over and became mostly submerged in a creek in Gibson, Pa. When the second passenger was unable to exit the car, Andrew stayed with her and used his legs to keep her high enough so she could reach the air pocket. Thankfully, she lived and unfortunately, Andrew did not. Andrew’s character and courage saved her life. To honor Andrew, we created the Diesel Jam Truck Show in his name. This event now supports 20 scholarships to local high school students, who are entering a trade program as well as students already in Trade Schools. Andrew Mazza Foundation supports local communities through charitable donations and through our annual Truck or Treat Halloween event in Clifford Twp. Edrene Wright Scholarship - Margery Scranton Council of Republican Women of Lackawanna County
The Margery Scranton Council of Republican Women of Lackawanna County is again seeking a female student for its $750.00 Edrene Wright Scholarship Award this spring. We will be pleased to award one $750.00 scholarship in May this year.
Eligible college bound female seniors must reside in Lackawanna County, have at least a 3.0 GPA, show an interest in government affairs or politics and if aged 18 or older be a registered Republican.
If the female applicant is not yet 18 and thus not able to register as a Republican, it is imperative that one of her parents or legal guardian be a registered Republican.
Interested young women are encouraged to submit an application that includes a typed essay postmarked NO LATER THAN MONDAY, APRIL 10th, 2023.

Susquehanna County Farm Bureau Scholarship
The Susquehanna County Farm Bureau is proud to offer two, $1,000 scholarships to qualified senior students in our county schools. All seniors in Susquehanna County schools are encouraged to apply. A presentation of the award will be made at the school district’s discretion such as Senior Class Night or the graduation ceremony. However, the specific cash award will not be given to the student until they successfully complete the first semester of post high school matriculation.
We are interested in a student’s entire school and community involvement along with each individual’s plan for growth after graduation. We would like to offer the scholarship to students as a way to encourage a long-term interest in agriculture and agriculture related fields and its positive impact on our communities and way of life.
Winners will be selected from the completed applications received by the deadline of April 1, 2023. Winners will be selected utilizing a scoring combination composed of points awarded in different categories such as academics, school and community involvement, career goals, and an essay.
2023 Jeremy Clark Memorial Scholarship

· Must have taken building courses at the Lackawanna CTC (Masonry, Carpentry, electrical etc)
· Must want to further their education in their field or in other fields by going to college, another trade school, or taking on an apprenticeship role/ internships.

Helping Hands of Susquehanna Scholarship

The Endless Mountains Primitive Outdoorsmen (EMPO)
Eligibility:
1. Students must be pursuing a degree or certificate at an accredited institution of higher education (i.e. Junior College, Community College, Technical College, College, University or Trade School)
2. Students must be involved in school and/or community activities. (i.e. FFA, 4-H, Ecology Club, Scouting, Science Club, Student Council, Leo Club, Leadership Council, etc.)
3. Students must submit a 700 - 1000 word essay responding to ONE of the three prompts listed on page two. Your essay should use Times New Roman, 12pt font, and be double spaced. Your essay must include verifiable citations. The citations may be in your preferred format.
4. A letter of recommendation must be attached to the application from a teacher, mentor or other community leader.

Pleasant Valley Grange
Wayne County Community Foundation is offering the annual Pleasant Valley Grange Scholarship totaling $1500 to a student(s) to further his/her education. Applications will be accepted between February 1st to April 1st, 2023.
Eligibility for applying for this scholarship is as follows:
The applicant must reside in Wayne County
The applicant must currently be enrolled as a senior in high school.
The applicant must state reasons for applying for this scholarship.
Preference will be given to applicants that are planning on a future in agriculture or some related field such as, but not limited to: Forestry, Environmental science, Veterinary medicine, Veterinary Tech, Technology program, Natural gas industry.
Human Resources Foundation Scholarship
Eligible students will reside or attend high school in Wayne, Pike, or Monroe County. He/she will be planning to pursue a career in Human Services, Social Work, or Special Education. Preference will be given to individuals who are looking to pursue human, social, educational, or health/clinical career paths working directly with individuals with disabilities.
The student will demonstrate scholastic ability, financial need, and possess the attributes essential to pursuing a professional career path in the human services field.

PROGRAM OUTLINE
BACKGROUND
Mike Munchak has been teaming up with the United Way annual charity golf classic for the past 26 years serving as our tournament host and NFL Celebrity for nearly the entire time. In 2002 the event was changed to the Mike Munchak Charity Golf Classic and continues to be one of the premiere golf events in our area each season.
In honor of Mike's commitment to our community and the United Way of Lackawanna, Wayne & Pike Counties, during the 2011 event it was announced that a special Mike Munchak Community Services Educational Scholarship is being formed with the initial scholarship awarded at our event in June 2012.
PROGRAM
The United Way of Lackawanna, Wayne and Pike Counties has established the Mike Munchak Community Services Scholarship to assist high school students from Lackawanna, Wayne or Pike Counties, PA who plan to continue their education at an accredited university, college, or trade school in a two or four year degree program.
To be eligible candidates must be participants in programs funded by United Way of Lackawanna, Wayne and Pike Counties, have a demonstrated commitment through community service activities and be full-time residents and/or students of a school district within Lackawanna, Wayne or Pike Counties. Additional requirements including academic achievement, agency recommendation and a track record of community service as described in the section entitled "Eligibility".
The scholarship will be awarded for a one year period. This is a one-time scholarship for graduating seniors and is not renewable beyond this one year period.
The Award recipient will be chosen by the Scholarship Selection Committee and all decisions of the Selection Committee are final.
The scholarship will be awarded annually at the discretion of the United Way of Lackawanna, Wayne and Pike Counties pending the availability of funding. Funding for the scholarship will be made available from the proceeds of the Annual Mike Munchak Charity Golf Classic to benefit United Way of Lackawanna, Wayne and Pike Counties.
AWARDS
If selected, the scholarship recipient will receive $6,300.00 for the 2023/2024 award.
A formal awarding of the Scholarship will take place at the Annual Mike Munchak Charity Golf Classic dinner. The 2023 dinner is scheduled for Monday, June 26, 2023 at the Country Club of Scranton. Photos of recipient and parent (s) will be taken with Mike during the program portion of the tournament. A current photo release must be on file with the United Way prior to the event and is included in the application packet.
Awards are granted without regard to race, creed, color, religion, sexual orientation, age, gender, disability or national origin.
ELIGIBILITY
Applicants for the Mike Munchak Community Service Scholarship shall:
• Be graduating seniors from a public, private or charter school in Lackawanna, Wayne or Pike Counties, PA and/or be full-time residents of Lackawanna, Wayne or Pike Counties, PA.
• Have demonstrated community service activities within programs/agencies funded by United Way.
* Meet minimum academic requirements (GPA of 2.5 or better).
• Submit a valid FAFSA Score (EFC Number). Copy of the page indicating score must be attached.
• Be accepted by an accredited university, college or trade school in a two or four year degree program.
• Complete and submit a valid signed application form along with all required documentation, essay response and letter of reference from the nominating United Way funded program/agency.
APPLICATION
Interested applicants are required to complete the application, enclose all required documents of support including your essay and letter of recommendation and mail to the United Way of Lackawanna, Wayne and Pike Counties, ATTENTION SCHOLARSHIP COMMITTEE, PO Box 526 Scranton, PA 18501-0526.

Applicants will receive acknowledgement as to the receipt of their application on a rolling basis.
Applicants will be evaluated on the information supplied. Incomplete applications will not be evaluated. All information received is considered confidential and will only be reviewed by the Scholarship Selection Committee and appropriate staff support.
PROGRAM/AGENCY LETTER OF RECOMMENDATION
All applications must include a written recommendation from the Executive Director or their authorized official Agency Staff Representative documenting community service involvement and reasons applicant should be selected. The recommendation must be from a program/agency funded by United Way of Lackawanna, Wayne and Pike counties.
SELECTION OF RECIPIENTS
One recipient will be selected in any given year. The award will be for one year only.
Recipients will be selected from a pool of qualified applicants with consideration given to those who demonstrate an extraordinary involvement in community service with a United Way funded program/agency, past academic performance and future potential, statement of career goals, unusual personal family circumstances, response to the essay question and stated recommendations of program/agency Executive Director or their authorized official Agency Staff Representative.
Selection of recipients is made by the Scholarship Selection Committee. At no time will any officer or employee of the United Way of Lackawanna, Wayne and Pike Counties be part of the selection process. All applicants, in submitting a signed application, agree that the decision of the Scholarship Selection committee is final.
EXCLUSIONS
United Way staff, board members and their immediate families, the Scholarship Selection Committee and their immediate families are not eligible to apply.
PAYMENT OF SCHOLARSHIPS
United Way of Lackawanna, Wayne and Pike Counties will pay the scholarship in TWO equal installments mailed directly to the college, university or trade school with the checks made payable to the school in the name of the student. The scholarship recipient must furnish the official name, address, phone number and point of contact to the United Way for the check to be posted.
The first payment will be released in August once written notification of acceptance from the school is received by United Way. The second payment will be released in January upon written notification from the recipient demonstrating they maintain a full-time status with the school.
At no time will the scholarship amount exceed the total cost of tuition, books and room and board.
OBLIGATIONS
The recipients of this scholarship shall have no obligation to the United Way of Lackawanna, Wayne and Pike Counties, its funded programs and/or agencies. They are, however, required to notify the United Way of any changes in address, school enrollment or status, or other relevant information that may affect the intended utilization of this scholarship.
The recipient agrees that the United Way of Lackawanna, Wayne and Pike Counties may use his/her name and/or photograph for publicity purposes upon acceptance of this scholarship without further notification or compensation.
This scholarship shall become null and void and no funds shall be tendered if the selected recipient violates any of these agreements, including withdrawal from school or dropping below full time student status.
REVISIONS
The United Way of Lackawanna, Wayne and Pike Counties reserves the right to review the conditions and procedures of this scholarship program and make changes at any time including termination of this program.
All applicants understand that this is a one-time one-year scholarship and is nonrenewable. The availability and amount of the Scholarship will be determined annually based on available funding.

The Scholarship Selection Committee has the sole responsibility for selecting recipients based on the criteria set forth herein.

Wayne Memorial Hospital Auxiliary Scholarship
The Roy W. Piper Charitable Trust
The Roy W. Piper Scholarships are administered through the Roy W. Piper Trust Scholarship Administrators (Ronald G. Kukuchka, Thomas P. Tulaney, Robert P. Browning, and William A. Petty). All applications can be requested from the Trust or from the student’s school guidance office. The Roy W. Piper Charitable Trust will accept applications by mail, addressed to: P.O. Box 460, Tunkhannock, Pennsylvania 18657. Only fully completed, hard copy applications will be accepted. Incomplete applications may, in the discretion of the Scholarship Administrators, not be considered. Electronic submissions will not be accepted.
2023 Blair E. Caboot Memorial Scholarship
The scholarship applicant should excel in mathematics in high school and have future plans in a mathematics-related field.

Agriculture, The Heart of a Growing Wayne/Pike County Scholarship
The Wayne/Pike County Farm Bureau offers the “Agriculture, The Heart of a Growing Wayne/Pike County” Scholarship to a deserving high school senior as a way to heighten a long-term interest in agriculture and it’s varied positive impacts in our area. The Local Farm Bureau also hopes to promote student consideration of the growing field of agricultural career opportunities.
**Applications must be turned in to the Counseling Office prior to March 1, 2023.**
2023 Perry Westgate Memorial Scholarship
Amount of Scholarship $1000.00
Contest is open to any student entering a 2 or 4 year college under an agricultural, conservation, or environmentally oriented college curriculum. If there are any questions regarding the curriculum you plan to study, contact the office. Some examples would be Animal Husbandry, Agricultural Engineering, Aquatic Biology, Soil Sciences, Parks and Recreation, Forest Sciences, Agronomy, Wildlife Biology, etc.
STUDENT MUST BE ENTERING FIRST YEAR OF COLLEGE IN CURRENT YEAR.
The contest is open to Wayne County students only.
There is no discrimination to race or sex.
The scholarship is for one payment of $500 the first year of college and one payment of $500 the second year of college. Payments will be made directly to the school that the winner is planning to attend. Before the second payment is made the winner will be responsible for submitting a copy of the first year transcript and a confirmation from the college that the student is still enrolled in a qualifying curriculum.
The application is to be completed to the best of the student’s ability.
One letter of reference must be included with the application. The reference may not be a teacher or family member.
A one page, double-spaced essay describing future goals and objectives must be submitted with the student’s application.
A transcript of the applicant’s educational record must be included.
The school’s guidance counselor or principal must sign the application for the contest.
The judging is completed using the application and material submitted, and how much the activities and information given pertain to the college curriculum that is chosen.
The last day to submit applications is THURSDAY, APRIL 6, 2023.

Forest City Mask and Drama Club Scholarship
- All applicants must be a 12th grade student who is a member of the drama club in good standing.
- All applicants must have been a member of the drama club for either two full years OR four total performances during their 9th, 10th, 11th, and 12th grade years.
- All applicants must be attending a college, university, trade school, or any other institute of furthering education in their chosen field of study.
Daughters of the American Revolution (DAR) Scholarship
Students should submit completed application to their School Counselor by April 1, 2023.
United Steelworkers Scholarship Program
Eligibility
An applicant must be the son, daughter or legal ward of a member of the United Steelworkers, District 10 (Eastern) in good standing with the Local Union participating in the Scholarship Program; or the son, daughter or legal ward of a disabled or old age retiree or deceased member of an affiliated Local Union, who was in good standing with the United Steelworkers at the time of their retirement or death.
