Semester 2 Learning Commitment Form
It is time to indicate your choice for your student's semester two learning environment. EVERY FAMILY needs to submit one response per student, regardless of whether or not you want to change your student's current enrollment. The form will be open for responses until November 2. Please make sure you submit one response for every middle school student in your family no later than that date.

We will adjust our master schedule for semester 2 based on these survey responses. Liberty has been as understanding as possible, based on our numbers, to making adjustments after the first semester deadline over the past nine weeks. We will not be able to continue this leniency as we head into semester two, so please be certain of your family's choice.

9 Week Progress Reports
Progress Reports for the first quarter were posted in Campus Portal on Friday, October 23. Please LOGIN to Campus Portal to check your student’s 9-week grades and be aware of the following as we begin planning for next semester:
There is NO hold harmless provision this year. Students will only have the remaining 9 weeks to be in good standing going into next semester.
First semester grades will be averaged with second semester grades to calculate the final grade for the year in core, academic classes.

Attention Veterans & Families of Veterans
This coming Veteran’s Day, Liberty Middle School would like to honor the active service members and veterans in our community. We would like to recognize their patriotism, sacrifices and service to our country. We are asking our Liberty families, staff members, and community members to visit the link below and fill out the form. You will also be asked to upload a photo of the active service member or veteran to be shared with our students and community on Veteran’s Day.

Our football season came to a close last Tuesday, as our Patriots played their hearts out against South Forsyth Middle in the first round of the playoffs. While we did not get the ending we wanted, we are so proud of the growth our athletes showed over the course of the season. A BIG THANK YOU to our coaches for their endless dedication. Also, much appreciation to our parents and fans for the support you have shown all season!

PTSO Corner
Our PTSO goes above and beyond to support our staff, students, and community! Last week, they helped us celebrate our bus drivers for Bus Driver Appreciation Week and provided snacks for our teachers during conferences. Thank you to all of our members who make these tokens of appreciation possible; they mean so much!

Notes from the Patriot Clinic
8th Grade Vision Screening will be conducted in RED day Connections classes the week of October 26th, November 2nd, and November 9th. Please make sure your student wears their glasses or contacts to school.
GA Form 3300 (Hearing, Vision, Dental, Nutrition Screenings): Parents of new students to Forsyth County Schools who have not completed all or part of their GA form 3300 for the school year will be able to complete these screenings for FREE through services provided by Forsyth County School nurses from 9 A.M. -12 P.M. on Saturday Oct. 17th (last name A-G), Saturday Oct. 24th (last name H-O), Saturday Nov. 7th (last name P-Z). If you have not completed your GA Form 3300 please call or email the school nurse for more details regarding the location of this clinic.

Attendance
- Attendance will be taken daily as usual.
- If your child is absent, please send in their excuse note or doctor's note within 5 business days.
- Feel free to contact teachers or Danielle Worley (dworley@forsyth.k12.ga.us) to notify them of an absence, especially if you anticipate your child being out more than one day.
Students Attending Forsyth Virtual Academy online
- Attendance will be taken weekly, based on work completion.
- As long as your student is logging into ItsLearning regularly and completing work in a timely manner, they will be marked present.
- If you receive a notification over the weekend that your student has been marked absent for one or more days, it means they did not submit all of the assignments for the week.
- If your student is too ill to log into itsLearning to complete work, please notify their teacher and Danielle Worley (dworley@forsyth.k12.ga.us) to make them aware of the absence.

COVID-19 Quarantine Communication Plan
General Health Precautions
- Hand sanitizer stations are located throughout the building, but we encourage students to bring their own, as well.
- Our custodians will be sanitizing classrooms every afternoon. If you want to send cleaning wipes with your student to wipe down surfaces before use, please just make sure they are not bleach-based products.
- It is the expectation of staff and students to wear masks, if they are able. We want to be able to maintain face-to-face instruction all year. In order to do so, we MUST work to keep each other safe; mask-wearing is a significant part of this.
