ORHS NEEDS ASSESSMENT FORM
Throughout the year different organizations come to Oak Ridge wanting to assist our students and families with school supplies, clothing, and other personal needs resources. Under the Family Educational Rights and Privacy Act (FERPA), which protects the privacy of student educational records, we are prohibited from releasing any student information to these agencies without your permission. The programs that we partner with vary from year to year to assist our students in some capacity. By giving us permission to submit your information to these programs, it does not automatically mean you will receive assistance. Most programs are limited and are based on the resources available. If your family or child is in need of assistance please complete the following form.

We can all help prevent suicide. The Lifeline provides 24/7, free and confidential support for people in distress, prevention and crisis resources for you or your loved ones, and best practices for professionals.
Crisis services can be accessed through any of the mental health clinics in all 3 counties. For crisis services individuals should contact the crisis line or call 911 (for life-threatening emergencies).
