FRESHMAN/TRANSFER STUDENTS-PHYSICAL & DENTAL FORMS
All incoming Freshmen and Transfer students are required to submit a State of Illinois Certificate of Child Health Examination with proof of immunizations, a Dental Examination Form and an Emergency Information Form. Failure to have these documents submitted online before October 15, 2022 will keep students from being able to attend classes or participate in activities until the documents are submitted.

Make an appointment with a CVS, Walgreens, or Osco location that administers vaccines.
Health documents MUST be submitted online. We will no longer be accepting forms dropped off in person, mailed in or sent by email. Once your forms have been reviewed by the school Health Services Office, you will get an email indicating whether or not the forms were approved or rejected.
