In order for students to participate as soon as possible, the following forms must be completed for any student who has not played in the past. All athletes need a current physical.
1. A current physical - must use this form, and must be filled out by a doctor - any other will not be accepted: Must be signed by MD or DO
2. Concussion form - signed electronically
3. Consent form - signed electronically
4. Dragonfly Account - A dragonfly account must also be created for each student. Dragonfly is the online database that AHSAA is using to keep up with player documentation. This is where the documentation will be kept up to date and where the concussion and consent forms can be signed electronically. Click the link below for sign up info.
For all of the parent meetings that are upcoming, we're asking for only 1 parent in attendance (the athlete doesn't need to attend) in order to maintain social distancing. Thank you!
