How to check out an iPad from the OTHS Library

Please read the following information and sign the Student/Parent Equipment Use Agreement google form below giving your child permission to check out an iPad. Once we receive the completed google form, the library staff will follow up with a phone call to the parent to verify student permission. Once permission has been verified, students may check out iPads through the library. These iPads must be used for classroom assignments and can only be checked out for 5 school days at a time. Students with overdue iPads will be assessed a fine of $1.00/day.
Check Out Procedures
*Students and parents must have signed the agreement PRIOR to check out.
*iPads are for homework and study purposes ONLY.
*Students may only check out the iPad for 5 SCHOOL DAYS at a time.
*Students will return the iPad to the library.
Responsible Use & Proper Care
*Students are not to make any changes to the settings. This includes, but is not limited to: account changes, email settings, wallpaper & device name.
*All pictures and videos taken with the iPad should be for school/educational purposes.
*All images downloaded to the iPad should be for school/educational purposes.
*iPad case should be left on at all times.
*The iPad should be stored in a safe place when not in use.
*Siblings should not be allowed to use the iPad.
*Chargers will be going home with students.
*The cost for damaged or lost iPads/accessories is posted below.
*If an iPad is lost, email Ms. Tuttle immediately at: nataliemtuttle@katyisd.org.
Returning iPads to the Library
*Delete all pictures and videos.
*Close programs running in the background.
*If student returns iPad AFTER 5 school days, student will be assessed a $1.00 late fine per day.
