End of Year Closeout Notes

  • RP4 progress reports should be updated and finalized for all students. One copy should be sent home, and one copy should be placed in the special education file.
  • Case Managers who serve students who are on academic consultation should have the completed Option 1 Consultation Form placed in the special education file.
  • Teachers who serve students who are transitioning to a new school (e.g. From middle school to high school) and who have adaptive equipment should label the equipment with the student’s name and receiving school.
  • Case Managers who serve students who have Behavior Intervention Plans and who are transitioning to a new school (e.g. From middle school to high school) should compile a list. The list should be given to Sped Clericals before May 26th. District Office will pick them up on the 27th.
  • AHS Case Managers who serve students who are exiting should complete the Summary of Performance on each applicable child and finalize it in Easy IEP. One copy should be sent home, and one copy should be placed in the special education file.
  • All assistive technology student devices must be turned into the sped clerical at each school on May 27th.
