2019-2020 School Health Requirements
If you have a student attending Murphy Junior High during the 2019/2020 academic year please review the following health requirements. If a student does not submit proof of meeting these requirements they will not be allowed to pick-up their schedule and will be excluded from school until documentation has been received in the school health office.
*Appointment cards and notes from a physician indicating future appointments and immunizations will not be accepted unless there is a documented medical exemption.
An annual health history form is required for all students and must be completed prior to the start of each school year. The information is shared with paramedics in the event of a medical emergency and it is important for health services staff to be aware of current medical diagnoses and needs. See attachment.
A physical examination as prescribed by the Illinois Department of Public Health and the Illinois State Board of Education is required for all students entering 6th grade. The physical needs to be dated on or after August 14th, 2018.
Any child entering 6th grade shall show proof of having received one dose of meningococcal conjugate vaccine on or after the 11th birthday, and one dose of the Tdap vaccine, after age seven.
All students entering 6th grade are required to present proof of a dental exam before May 15th, 2020.
Please submit all forms and documentation to the Murphy Junior High School Health Office in person or via fax or email. Feel free to contact the nurse with any questions or concerns.

Ms. Schultz - Math
The students measured their feet to the nearest 1/2 an inch and created a class line plot. After, we discussed how to create and interpret line plots. Then, each group had a discussion using level 2/3 questions which helped interpret the data further. Finally, they shared their groups perspectives in a stand-up, hand-up, pair-up activity.This week I randomly paired students up. They had to ask students questions to learn more than surface level things about them. They then needed to use higher level academic vocabulary to describe the student. They wrote and acrostic poem about the student with the additional requirement to add images and to share with the class the words they chose to describe the person and why. They did an excellent job!

Murphy Junior High students are doing amazing things in the classroom as well as demonstrating “The Maverick Way” outside of school. Track
Nice job Murphy track at conference on Friday!
A special congratulations goes out to the 8th grade boy's track team on getting first place at the Quint!
Highlights include a 5th place finish in the hurdles by Lukas Gacek, the mile by Zachary Benoit, the 200 yard dash by Zaccari Starkman, the 800 by Jared Tam, and the long jump by Aidan Murray.
All three relay teams finished third. The 4x200 relay team included Ayden Day, Mekhi Fikremariam, Aidan Murray, and Basir Muhammad. The 4x100 relay team consisted of Benji Paranidharan, Mekhi Fikremariam, Nart Gjinolli, and Basir Muhammad. The 4x400 relay team consisted of Ethan Gustafson, Collin Basken, Jared Tam, and Alex Das.
Zaccari Starkman placed 3rd in the 400. Athletes who earned a second place finish were Basir Muhammad in the 100, Jared Tam in the 400, Alex Das in the 800, and both Lukas Gacek and Nart Gjinolli tied for second in the high jump. Alex Das got first place in the mile with a time of 4 minutes 57 seconds. Basir Muhammad received first in the 200 meter dash with a time of 25.27 seconds. Mekhi Fikremariam earned first place in both the shot put and the discus with throws of 36 feet and 102 feet 6 inches respectively. School records were made or tied in the mile, 4x100, 4x200, 200 meter dash, discus, and high jump!
Excellent job gentlemen, and thank you for an amazing season!
