Details:
You have been invited to complete the Illinois 5Essentials Parent Survey. The information you provide will help guide improvement as we strive to inspire each student to realize their potential. Your participation in this survey is voluntary but crucial. We can’t receive results from the survey unless at least 20 percent of parents respond. The more responses parents provide, the more we will know how to focus our efforts toward the improvements you want. Therefore, we have set a goal to reach an 80 percent response rate. Please help us reach our goal by completing the survey.
· Go to: https://survey.5-essentials.org/Illinois/ to take the survey.
· Select “Parent Survey.”
· You may enter your email address if you would like to receive confirmation of your submission, but it is not required.
· Then enter “Cook” into the county field and “Lincoln” to find our school.
You will then be asked a series of questions to provide input regarding your family’s experience here at Lincoln. Please remember to click “submit” once you are done so your responses can be counted.
Please know that your identity and responses will be kept confidential and will not be connected
with your child. If you have multiple children who attend Lincoln, you may complete one survey for each child. However, only one parent per household may submit a survey for a given child. The survey closes on March 11, so please submit your responses soon!

February 23rd Changes to Lake Louise School Times
The district has shared, that due to a bus driver shortage, we are moving to an adjusted time schedule on February 23, 2022. Lake Louise's school hours will be pushed back.
Remember the below times BEGIN ON FEBRUARY 23!
- Doors open at 9:00 am. Arrival supervision begins at 9, so please do not drop children off before 9:00 am.
- Dismissal M - Th: 3:55 pm
- Dismissal Fridays: 3:05 pm
- The Lake Louise office will be open from 8:15-4:45.

District 15 Updates
Online Registration for the 22-23 School Year
Online student registration for the 2022-23 school year is now open. If you have not already started this process for your student(s), please visit our registration page to start this process.
Registration is the first step in getting ready for the up-coming school year. After you register your child online you need to:
Pay student fees. Our student fee webpage outlines the District’s student fee structure and the three ways fees can be paid.
If you have a student that is starting Kindergarten, entering fourth grade, or entering 7th grade, you are required to prove residency in District 15. Our residency webpage outlines the documents you will need to provide to prove residency.
All three steps of the enrollment process must be completed prior to the start of the school year. Students of families that, when required, do not prove residency, will not be able to attend class in a District 15 until the family’s residency is verified.

Orland Park Dental Services for CCSD15 Students
Community Consolidated School District 15 will be hosting Orland Park Dental Services (OPDS) coming up in March through May. OPDS has provided in-school dental services for all eligible children in the State of Illinois since 1999. Dental services will be accessible to children eligible for public aid as well as for those with private insurance.
Anyone signed up will receive a free dental exam and a toothbrush. For those that qualify or provide private insurance information, the additional services may include a cleaning, fluoride treatment, and sealants. All services done will be given to the child in a take home form describing the services determined, areas of concern, overall oral care score, and any additional care needed. If any additional help is needed for the child a comprehensive list of available local dentists will be given with no limits of insurance. All these in-school services will be provided by licensed dentists, hygienists, and assistants with state-of-the-art portable equipment and ensuring a clean, safe, and comfortable environment at your child’s school.

Parent/Guardian Contact Information
Parents/Guardians - Please make sure you have all of your contact information updated in Infinite Campus (email address, phone number, and cell phone number) for absence calls, school, District, and Board newsletters, emergency notifications, etc.
To check your contact information, you can login in Infinite Campus to review and update.
Mobile App
The District 15 mobile app is available on the App Store and Google Play! To download the free app, search for "CCSD15."
This communication tool allows parents and students to quick access to important school and district information, including Campus Parent, the D15 Bullying Tip Line, a quick link to MyStop (bus stop location and pick-up times), lunch menus, schedules, parent/student handbook, directory information, school news, and more.
To see integrated student information on the app, click the "student information" button and log-in using your email address and your Campus Parent password. If you are unsure of your password, please visit Infinite Campus and click "forgot password." If you are still having trouble, please contact your school's main office for assistance.
To make sure you aren’t missing any notices, make sure you turn notifications ON for the mobile app.
If you are not a D15 parent or staff member and would like to receive District updates, download the free mobile app to receive push notifications.
Go to Notifications, Settings, and choose the schools you wish to receive push notifications from.
