So grateful for all of you do! There is much work to be done: evals, ICs, panorama feedback, and checking how your leadership is impacting Culture & Climate and Academics as we are coming upon March. This critical work is best done when you bring your best self forward. Mike and I are proud of you and the work being done on behalf of learners in our community.

We know that you are all busy, but if you could take a few minutes to fill out this google form concerning the Ready.Set.Supply! (RSS) program, it would be appreciated.
This will be our third year with the RSS program and we are constantly wanting to improve on our process. We asked for feedback from the partners through a Thought Exchange, so now we would like to hear from our Principals and/or any other staff you would like to pass this form on to, to fill out. All feedback will be welcome.
We will be gearing up for RSS here within the next month so your feedback is important to share in the upcoming meetings.

Online 504 Plan Requests
Principals, Assistant Principals, Counselors, and Nurses: as mentioned in our recent meetings, here is a link to the new 504 online referral form. This link is also posted on the Special Services page and on the Parent Resources page of the SPS website.
Should a parent approach you about a 504 referral, please direct them to this link, which will provide us with the information we need to proceed, as well as allow parents to attach/upload medical documentation to support the request. If parent does not have the medical documentation at the time they complete the form, it can be scanned and emailed to 504referral@spsmail.org . Upon submitting, parent will receive the auto reply shown below.
For parents without computer access or the requisite technology skills, a hard copy of the referral is attached which can be printed. Hard copy referrals and medical documentation can be sent through school mail or mailed to the Bentley Administrative Center at 1610 E. Sunshine (65804), c/o 504 Referral.
Auto Reply Upon Submission of Form:
Thank you for contacting the Special Services Office regarding a 504 referral for your student. The 504 Procedural Safeguards are attached to this email. A 504 process coordinator will be in touch with you within 30 days. If you have questions, please email us at 504referral@spsmail.org or call the Special Services office at 523-7500.
Follow up questions to the new 504 plan request process:
1. So it is preferred that the parent complete the request. Correct?

2. How will the school be notified a request has been submitted?
The request form feeds into a database that is checked daily by one of our secretaries – she makes the process coordinator aware to make contact with family and review the info. If it is a legitimate request, process coordinator will notify school so a meeting can be set up
3. After a request is submitted, how long is the expected time we should wait to hear back and have a meeting scheduled?
We are mirroring IDEA timelines, so process coordinator has 30 days to review information and consider request – the meeting can be set up after that occurs.
4. Will we still have our same assigned PC for 504?
Yes, at least for the remainder of this year

SLP Evaluations: The Special Services department has taken the Speech Language Pathologist (SLP) evaluation off your plate, but hoping to include your feedback in the process.
The SLP growth plan/evaluation form is in TOWER and has a section titled Site Administrator Feedback. We discussed the change at BOY meetings, and the process is very similar to your feedback on school nurses.
You may consider meeting with your SLP to review their performance from your perspective, either solo or along with Pam Marion. Each SLP will meet with Pam, Assistant Director of Therapy and Related Services, to review performance and professional development goals. These meetings will occur throughout February and March, and Pam will let you know when she will be at your site. Please do not close the evaluation form with your PIN number; the evaluations for the SLP’s will be completed by the Assistant Director of Therapy and Related Services.
