The process is very simple and will only take a few minutes. The form can be filled out multiple times, so please continuously update your child's information as more scholarships are awarded.

NEW ** ATTENDANCE - 317-803-5535
Students are to bring a note from their guardian/parent to the attendance office or the FA office before school for their pass.
If students are sick and need to leave immediately, they should go to the nurse.
If you are dropping your student off late, you do not need to come in with them, but you do need to let Attendance office know, either with a note or phone call, why your student was late. The student must sign in on the chrome book in either the FA office or the Attendance office to be counted as present for the remaining classes.
The attendance line accepts messages 24/7.
Please contact the Attendance Office, not the Receptionist, and leave a message for anything pertaining to your student's attendance. You must report their absence whether they are late, leaving early, or going to be absent for the day. The person reporting the absence must be the same person listed as guardian in Infinite Campus. Even if your student is 18, you must report their absence.
If you are picking up your student early from school you must send in a note with your student (preferred method) or call the Attendance Office, leave a message with a specific time for pick up, and come in to the school to present your Drivers License Identification to the Receptionist.
Please do not call the Receptionist, Clinic, or Guidance Office to send your student a pass to leave early. Even if you are on your way, please call the Attendance Office so they can send your student a pass.
If you receive the automated call, the caller ID will show the main school line. Please DO NOT call back to this number...call Attendance at 803-5535. The listed guardian must report absences or early dismissals. If anyone other than the guardian reports the absence, it will not be noted in Infinite Campus. Please make sure you, the guardian, return the call to the attendance line at 803-5535
If you have questions about a recorded absence in Infinite Campus you can contact our attendance office or email Mrs. Bailey (courtney.bailey@ftcsc.org).
