Welcome Back from Winter Break
COVID Testing Kit Reminder
On Monday, December 13, you received an email letting you know that your student would receive a free COVID-19 Antigen Rapid Test Kit today, Thursday, December 16, intended for use prior to returning to school from Winter Break.
Included below is additional information and guidance on the timeline for test use, how to administer the tests, and guidance on how to report results. Use of these test kits is optional.
When to Use Tests Kits
Each kit contains detailed instructions for self-administration along with materials for two tests:
Test #1 should be used 3 days before returning to school on December 31, 2021.
Test #2 should be used 1 day before returning to school on January 2, 2022.
How To Use Administer the Tests:
Test kits are to be completed at home by students (or with help from parents on students) and by school employees.
Detailed instructions are included inside the test kit; instructions in Spanish are attached below.

Reporting Test Results:
These test kits have been provided by the California Department of Public Health (CDPH) and in order to monitor the effectiveness of this proactive and preventative testing program, participants have been asked to report their test results on Primary.Health, not the iHealth app as indicated on the testing box.
If your student has participated in our on-campus modified quarantine testing program, they already have an account with Primary.Health.
If your student does not have a Primary.Health account, please create one by completing this online registration and consent form, as this will be needed to report their results.

Next Steps After Testing Completion:
If your student receives negative test results and is asymptomatic, they may return to school on January 3 as scheduled.
If your student receives one or more positive test results or is symptomatic, please refer to the ACPHD guidance and contact your healthcare provider.

Reminder: LVJUSD's Modified Quarantine Student On-Campus COVID-19 Testing
The Alameda County Public Health Department published a Quarantine Guidance document that illustrates the quarantine options for students who have been in close contact with a positive COVID-19 case.
To participate in LVJUSDās modified quarantine student on-campus COVID-19 testing, a parent or guardian must complete the registration and consent form. This form only needs to be completed for each student one time.
